Suggest Treatment For Severe Pain In The Ribs

default
Posted on Wed, 8 Feb 2017
Question: Dr. T. Chandrakant,

Sorry I didn't respond timely Doctor.
I apologize.

To answer your questions...
The pain is below the bottom right rib (see attached picture).

The pain can be (and is often) relieved when adding pressure to the area (often with using fingers applied).

Food: the pain does tend to be heightened when eating fatty foods (such as burgers or junk food), but I've noticed that it hurts regardless (even right now as I type), it's hurting quite badly.

The pain is a very strong ache (not so much a sharp pain). It's a very strong discomfort as opposed to a stabbing pain. It also has incited a bit of nausea but no vomiting at all.

I went to the er recently due to this pain, but the doctor did not want to run a ct (due to one I had 7 months ago that came back normal).
They drew blood related to the liver and pancreas and all came back normal. My GI doc will want to do an endoscopy at some point soon.

The pain is enhanced in the middle of eating a meal and after as well.

There is pain that will radiate to the back as well.

This pain has only gotten worse over time so it's getting quite frightening Doctor, considering that an er couldn't even help.

Brief Answer:
Please post another picture without clothes and mark the ribs

Detailed Answer:
After reading your feedback and original question deeply, I can think of Costochondritis as the most probable reason for your pain.
Gastric or Duodenal inflammation or ulcer as a second option.

You have put up the fingers at such a spot.
Please post another one with bare skin and please tell me it is just below the ribs- you may please mark the lower ribs. And whether the purposeful coughing or straining or moving in a particular position increases the pain.

Awaiting.

Yes it is just at/just below he ribs. Coughing/stretching/moving doesnt aggravate it. Sitting down does though.

I should maybe mention that i dont feel bloated particularly but i do sense a sort of tightness when pressing my stomach...considering how overweigjt i am, i didnt think i would feel anything tough if that makes sense .

Brief Answer:
Costochondritis

Detailed Answer:
Thanks.
Looks to be costochondritis as per the fingers you have put on
I would advise you the following in such a situation:
-Rest to the area by means of strapping with the Adhesive tapes during the deep Expiration phase. This will limit the movement, reduce the pain and give rest for healing to occur.
-Anti-inflammatory medicines in full dose as per the weight.
-Steroids if required for its strong anti-inflammatory role.
-Diuretics to reduced the edema.
-Control of stress and anxiety helps a lot.
-Local rubrafacient liniments if the area is not covered by strapping.
-Confirmation of the diagnosis by local Doctor on palpation and clinical evaluation and to get prescription based medicines after discussing the above points.

This sort of a treatment helps all my patients. and should help you too.

Would costochondritis cause intense pain in the stomach though? I have the pain/pressure in the right upper quadrant but also have this stinging pain in my stomach (or possibly intestine) now. This is really frightening and my gi doc appointment isnt for another two weeks, and the er couldnt do anything.

Brief Answer:
As discussed in details below.

Detailed Answer:
The pain of costochondritis can be simple discomfort to severe enough to mimic anything like heart attack and any problem in the stomach.
Sometimes it is very difficult for the patients to pin-point and explain the pains.
You can discuss about possible diagnosis of costochondritis with the Doctors at ER or your PCP/Family Doctor so that the process of diagnosis and proper management rolls on till to get to see the GI Specialist.
If this is due to costochondritis, there is nothing to frighten about as this is treatable.

Understood doctor. If this were to be either costochondritis or something visceral, would working out be ok, or would that cause harm?

Brief Answer:
Depends upon many factors.

Detailed Answer:
This would not cause harms if the instigating factors are avoided.
You would understand on working out whether some actions or movements cause any harm.
And also important is to get the correct diagnosis to decide whether working out would cause any problems or not.

Understood. My appointment with my GI doc is next thursday. My ultimate concern with this ongoing pain is whether or not its fatal. I hope that it isnt something cancerous, being that im only 30 years old. But considering, i cant get any result, its hard to not think about that ossibility. p

Brief Answer:
As discussed

Detailed Answer:
Since you are 30, the chances of this pain and its cause being fatal or cancerous is very very minimal.
Please discuss all the above points with your GI Doctor.
Let us hope of getting the final diagnosis on your visit to a GI.
Please give me the feedback.
