Suggest Treatment For Severe Nausea, Abdominal Pain And Lower Back Pain

default
Posted on Fri, 22 Jul 2016
Question: It started as severe nausea for a week, and then turned into abdominal pains on my right and left side. The past few days ive had lower back pain radiating up the right side of my back.

At the same time ive had no fever, or vomitting. I have had the urgency to relieve my bowels immediantly.

I have a history of ovarian cysts, and my doctor detects IBS.

Do you think this sounds like early appendictis or gallstones?

Brief Answer:
Does not sound like appendicitis or gall stones ...

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/24 - nausea for a seek - pain on both sides of abdomen - low back pain radiating up the right side of back - urgency to relieve bowels - history of ovarian cyst and IBS - wants to know whether this can be due to early appendicitis or gall stones ...

My thoughts:
As per the history you have provided, this can not be due to appendicitis or gall stones as the pain is on both the sides of the abdomen and there is referred pain which is not usually found in appendicitis or gall stones.
Nausea and urgency to relieve bowels immediately with pain goes in favor of gastroenteritis or due to IBS.
Ovarian cyst can cause pain only on sudden increase in size or twisting (torsion) and is sudden and severe.
I would advise you the following:
Clinical evaluation and examination.
Ultrasonography of the abdomen.,
Tests of stool, blood and urine.

I hope this answer clears your doubts, helps you to get the perfect diagnosis and get a plan for proper management.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.

I had a pelvic ultrasound as I do every three months to make sure there are no cysts present. I will hear the results soon.
As for the symptoms it is almost like achy indigestion feeling going into my upper back near my right shoulder and abdominal pain on my right side in the middle of my upper/lower right quadrant. Sometimes the pain can be sharp or dull.

Does this sound like it could be serious?

Brief Answer:
As discussed below.

Detailed Answer:
Thanks for your feedback.
Since you get the pelvic ultrasound every three months, what was the result of the previous one?
The type of pain that have explained now is suggestive of one of the following:
Can be due to gall bladder inflammation as said to have achy indigestion feeling going into upper back near right shoulder. You have also explained that the pain is on right side in the middle of right upper/lower quadrants which can be sharp or dull.
Such type of pain can be due to colitis of the ascending colon, related to the right kidney and so on.
Such pains should taken seriously until the diagnosis has been made and all the efforts to be done to find out the cause and get treated properly.

I hope this answer helps you. Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
