Suggest Treatment For Severe Burning Sensation In The Chest

Question: Hi.
Iam having chest burning on and off for past six months. Pain Started after my ureteroscopy done to crush kidney stone. There is slight increase or no pain with moment happens mostly after a meal, sometimes pain stays all day from morning. Majority of time i can lift weights and do jogging with no pain or some slight burning it stays left side of chest. All my ecg and tnt, echo came back normal, antiacids are not giving me relief. Done upper endoscopy which came back normal

Hi, welcome.
This is Dr Sameer, cardiologist from Fortis Hospital.

Cardiac pain is generally non localized & it is diffused which increase with exertion. So considering your pain & the negative TMT rules out cardiac nature of disease.
Their are other common causes of chest pain which include muscle pain and costochondritis which is a inflammation of joint between bone & cartilage of chest. Costochondritis can occur after ureteroscopy, so it is the most probable cause of your pain.
It is a very benign condition which can be easily treated with pain killers. We generally give Tab Ultraset twice daily for 7 days for such patients. You can also continue Tab Pantoprazole 40mg once daily before breakfast for gastritis as their is no harm in taking it & will decrease the gastritis caused by pain killer.
Also get a X Ray chest done to be on the safer side, which i believe will be normal.

Sometimes there is slight discomfort with exertion. But not significant enough to stop me doing work. Other times there is no pain when lifting weights or running. Sometimes i can feel burning sensation when i touch surface of chest. Also discomfort stops if i pull my shirt. My chest xray is wnl. Also iam 29 obese 108kgs with 180cm

Hi,
Welcome back
Echo is normal. X Ray in costochondritis is always normal.
As the TMT is normal cardiac cause of pain is ruled out. And if the pain is cardiac it will always occur after exertion not sometimes.
So it is most likely costochondritis. Take Ultraset & Pantoprazole as i advised earlier & you will be fine.

Ok sir, my other symptoms were headache when standing doctor rule it has tension headache. Most of my headache are gone but sometimes i get it when standing on hard surface or feeling stressed. I did try normal painkillers like paracetamol which didnt relieve my chest pain. And my final issue is diastolic bp which is in upper 90s.doctor told me reduce weight and check after 3months if i need to start using medicine since iam only 29

Hi,
Upper range of diastolic bp for your age is 90mm Hg. We generally don't prescribe medicine directly for borderline bp. We first tell him to reduce salt intake, decrease weight, avoid junk food & do some mild exercise for at least 30min for 5 days a week. This has to be tried for 3 months. If bp is not yet controlled, we than prescribe medicine. So you can also try this. In the meanwhile monitor your bp regularly at least twice daily in morning & night especially when you are relaxed.
For headache, it looks like a tension headache, but a neurological will be a better person to guide you.
