Posted on Thu, 9 Jun 2016
Question: I have suffered from GERD all my life since I was a little girl. Well I am suffering so much from GERD and acid reflux to where I feel full as soon as I eat a few bites. I puke in my mouth when I bend over. I get these sharp unbelievable pains on both sides(mostly on the right side) of my upper abdomen under my breast. I have had my gallbladder removed a few years ago. I have an unusual odor coming from my stomach through my mouth. I know something is not right. Can you help me figure this out. Most nights I have to sleep with my bed elevated. I also eat Gaviscon and Tums every day. I take Nexium every day too.

Brief Answer:
Dyspepsia/GERD

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

You have symptoms of Dyspepsia and GERD.
Please continue taking Nexium.

The fullness you get after you get after a few bites might be due to gastroparesis.
Get your blood sugar level checked.
You will get relief with medication like Reglan.

For complete relief, the following should also be done.

1. Weight reduction if obese
2. Avoiding alcohol and smoking
3. Avoiding coffee, citrus, chocolates, carbonated beverages, tomatoes and fatty food
4. Eating multiple smaller meals and going to bed at least 2-3 hours after food
5. Following some stress relief methods like meditation

Also get a Repeat Upper GI endoscopy done to rule out ulcer disease.

Regards,
Dr.R.K.

But what is the unbelievable bad pain that I get in my upper stomach when I stretch or try to clean house or exert myself, also when I twist slightly just to wipe my butt after using the restroom. It hurts so bad I can hardly breathe. Could this be a Hiatal or paraesophageal hernia?

Brief Answer:
Unlikely to be hiatal hernia

Detailed Answer:
Hi,
Paraesophageal hernia is very rare.
Sliding hiatal hernia which is more common usually does not cause any symptom.
When it becomes symptomatic, it causes symptoms like heartburn, discomfort behind the breastbone etc.

Pain in the upper abdomen is caused by issues like peptic ulcer, pancreatitis etc.
But the one that you get when you exert yourself, stretch or twist is usually muscle or nerve related pain
Physical therapy and tylenol might provide relief.

Regards,
Dr.R.K.
