Suggest Treatment For Red Circular Bumps On Thigh

Question: hi sir,
i am having reddish colour circular type thing on my thigh near penis.it happens on some other parts for some period and then vanishes,but on thigh its permanent.i gives itching feeling...its been more than a year...i just apply cream tyza-m...itsgives relief for sometime but after few days same problem repeats.kindly help me out.

Hello. Thank you for writing to us.

I have gone through your query and I have also reviewed the images.

You have fungal infection in groin. Fungal infection of groin may not resolve completely on topical antifungals alone and therefore both oral as well as topical antifungals together are preferable specially if skin involvement with fungus is extensive and recurrent.
You also have stretch marks in groin. These stretch marks are due to indiscriminate use of creams which contain potent topical steroids in combination with antifungals. These combination creams can cause thinning of skin and formation of stretch marks if they are used for long term due to the potent steroid content of these creams. Infact the cream which you have been using i.e tyza m also contains a topical potent steroid, mometasone furoate 0.1% in addition to the topical antifungal, terbinafine 1% and should not be used in groin for a period longer than 2-4 weeks as it may cause skin thinning and stretch marks if used longer than this period. Therefore I suggest you to stop this cream.
If i was the treating doctor i would start you on an Oral antifungal e.g either terbinafine tablet Or Fluconazole tablet.
Oral antifungals are more effective in controlling fungal infection as compared to antifungal creams alone.
Topically i would suggest you to use just a plain antifungal e.g terbinafine 1% cream Or clotrimazole 1% cream i.e without any steroid content.
In addition I also suggest you to take a tablet of cetrizine once daily for symptomatic relief from itching.

I suggest terbinafine 1% cream and terbinefine tablets for 4 weeks.

Detailed Answer:
Hi.

Stretch marks are irreversible.
Stretch marks are due to over use of steroid content present in antifungal creams e.g tyza- m and similar other products. They should never be used for a duration longer than 2-4 weeks in groin.
Regarding recurrent fungal infection I would suggest you to use plain antifungal cream e.g terbinafine 1% cream i.e tyza cream twice daily for 4 weeks, rather than using tyze-m cream.
I also suggest terbinafine tablet 250 mg i.e tyze tablets, once daily for 4 weeks.
A tablet of cetrizine 10 mg once daily will provide you symptomatic relief from itching.
Thereafter you must use an antifungal dusting powder e.g clotriamzole % dusting powder (Abzorb dusting powder) in groin folds regularly so as to keep groin folds dry and prevent a relapse of infection.
These are all prescription medications and should be used under supervision of a dermatologist and only for the duration prescribed. I suggest that you visit a dermatologist in your vicinity for the needful.
