Question: My wife hit the back of her head on a shelf on Thursday. Seemed fine after, but back of head was sore and minor headache. Yesterday back of head was still sore and a bit of a headache. Today she has pain and ringing in her ear, back of head still very sore. Bouts of being nausea but no vomiting and spells of dizziness. Also has a small fever of 99.1. She is also menstruating if that matters.

Hope the pain is bearable and she is not having any other symptoms except the ones mentioned by you

Head injuries need to be observed for the first 24 hours. The following are alarming symptoms (within first 24 hours) of a head injury that needs a doctors attention.

1. Vomiting
2. Seizures
3. Persisting neurological deficits (numbness is a neurological deficit)
4. Difficulty in maintaining gait
5. Vision problems
6. Increasing severity of headache

If any of the above symptoms have occurred and are present even after 24 hours then you need to see a doctor and get a CT scan brain done. If she is experiencing dizziness and nausea which is not subsiding even after 3 days, you may visit the ER. Fever is not significant but nausea and discomfort may be associated with menstruation in some women. The persisting dizziness needs to be attended to.

If necessary a CT scan of the head may be done after clinical evaluation to evaluate any injury inside the head.
