Suggest Treatment For Parapelvic Cysts

Question: I had an MRI in January and my kidneys show cortical and parapelvic cysts. Should I do something regarding these cysts. My doctors did not say anything.

Dr XXXXXXX I sent you the MRI report. I have had blood work recently and my creatnine, BUN, and EGR were normal. I have lower back pain but no history of kidney cystic disease in my family, although my brother has protein in his urine and has been on medication.

Thanks for attaching the photo of the MRI.

They have commented that there are multiple cysts in the kidneys, the fact that you have multiple renal cysts and also cysts in the liver and pancreas points towards autosomal dominant polycystic kindey disease.

Here how you should go about it

1. Get an ultrasound of the kidneys to confirm the number of cysts. More than 4 cysts on each side points towards ADPKD
2. Get a urine routine done. Infections are common with cystic diseases which will require treatment
3. I put my patients on ACE/ARB. Tab Telmesartan 40mg once a day to reno protection
4. Lumbar pain is the most common symptom in ADPKD, it will require treatment with analgesics like acetaminophen
5. Please watch out for blood in urine, it indicates cyst infection
6. There is no food restriction

Dr XXXXXXX In January 2014 I had a CT scan. The reading was: Simple cysts up to 3cm are noted at the upper pole aspect of the right kidney and smaller simple cysts in the left kidney. There is no nephrolithiasis or hydronephrosis and no active perinephric urine extravasation is seen.

Dr XXXXXXX I also sent you a copy of an ultrasoud from December 2013.

The kidneys are negative for obstruction. The right kidney measures 11.0 x 5.4 x 6.7 cm. these measurements do not include a superior exophytic 2.9 x 2.6 x 2.9 cm cyst. A second superior cyst measures 0.9 x 1.0 x 1.1 cm. the left renal measurements are 11.6 x 6.3 x 5.1 cm. a midportion cyst measures 0.8 x 0.7 x 1.2 cm. a second measures 1.5 x 1.3 x 1.3 cm.

These are simple cortical cysts which usually are present in individuals above 50 years of age. They are of no harm and benign. Do not worry about them. No treatment is required at the moment
