Suggest Treatment For Migraine

Question: I have been have migraines since I was about 17 years-old (30 year-female). When I was 19, my neurologist found something that worked (Topamax), now I hardly, if ever get a migraine. Last night I woke up with one of the worst ones I have ever had. It is afflicting the entire right side of my head, including my eye, nose, ear, and throat (right side only). My question is, do you think this will go away like my regular ones I used to have, or should I call my PMD in the morning. I don't have a neurologist anymore as she retired, so I am in the process of establishing a new patient/Doctor relationship. Of note, I had an MRI of my brain about a month ago related to adrenal insufficiency, and it showed "A solitary 6 mm focus of high signal intensity in the left temporal subcortical white matter" that was not there in 2014. I don't know what that means because they didn't explain it, or if it's even significant. Thank you.

I have been trying a heat pack on my shoulders/neck, cool pack on my eyes, Tylenol, ibuprofen, and a left over fioricet from when I was pregnant (three months ago).

Hello,
Migraine is a disease which persists throughout adulthood.
The episodic attacks do not require a prophylaxis treatment like Topiramate if not frequently.
The treatment at present is pain killers like Naproxen, Aceclofenac, paracetamol.
Other specific abortive drugs for migraine are Rizatriptan or sumatriptan.
The attack improves in 3 days, if not then consider neurologist opinion.
Take adequate rest, timely food, proper sleep and avoid fasting, sunlight, chocolate, excessive tea or coffee.
The Mri change is non specific white matter changes which can be found in migraine too.
