Hi dear,
I have gone through your question and understand the concerns.
Thick endometrium can be present in very early pregnancy, or endometrial hyperplasia, which is common in PCOS.
If there is any chance of pregnancy, then get a urine pregnancy test done.
If no chance, then the thick endometrium and pain lower abdomen is due to premenstrual phase, indicating that the periods will occur in next 3-5 days.

I have done my tvs back in march.. everything is normal except bilateral pcos and thick endometrium that time..from last 3 months i am facing frequent spotting problem.. but recently from past 9 days having cramps like menstural one but no period.. i had unprotected sex on aug 16-17 .. i have a irregular periods..

Hi dear,
frequent spotting can be due to local lesions in the cervix or vagina or lesions like fibroid in the uterus, and not due to PCOS.
I will suggest you to get an ultrasound done to confirm the diagnosis and get further treatment accordingly.
