Suggest Treatment For Low Endometrial Thickness While Trying To Conceive

default
Posted on Wed, 29 Jul 2015
Question: Hello doctor, I am 32.5 years old and my husband is 35.5 years old and we are trying to conceive for last 1.8 years. I have polysystic ovary and irregular periods. Apart from this all my and my husbands tests are normal (including thyroid, body weight, fallopian tubes, uterus, cavity, ovaries, sperm count for husband). I have done 3 IUIs but failed. So underwent 1 IVF fresh cycle which also failed (in this cycle 8 embryos fertilised and 2 B-grade embryos were transferred). After which my next 2 cycles for FET were cancelled due to thin endometrium (below 7mm), even though I took gcsf injections and progynova 2mg. In this cycle, I have taken 3 gcsf injections on day 4, 7 and 10. Also taking progynova 2mg (6 tabs daily) and susten 400mg twice daily. FET was done with B-grade enbryo and with 6.8mm endometrium thickness showing 3 distinct lining on day 19 (on day 16, E2 and LH report was 2264 and 9.16 respectively). Also in this cycle, scratching and sonohysterogrom was done on day 2 where cavity was normal.Wat are the chance for me getting pregnant in this FET Ivf? Any suggestion from you which can help me increase endometrium thickness to 8 mm incase I need to go for more FET cycles? Kindly advice

Brief Answer:
FET to be postponed till endometrial improvement

Detailed Answer:
Hello,

Thanks for your query.

I have read you query and I understand your concerns about possibility of FET.

Following is my reply:

1) Pregnancy chances are good if endometrium is more than 7mm with triple line character.

2) In your case if it is less, no use of FET. I suggest you wait for improvement.]

3) Request your doctor to add oentoxyphilline and Vitamin E to your prescription which will improve endometrium.

4) as of now it is not advisable to go ahead with FET.

I hope I answered you query and addressed your concerns.

Let me know if you wish to ask any further questions. I will be glad to help you.

Thanks for the revert doctor. The two prescriptions you have recommended to increase endometrium thickness need to be taken as oral medecine or through injection or vaginally? And on what days it needs to be taken and what should be the dosage and for how long? Also if any side effects expected from them. Kindly confirm. Also looking at my history as explained by me in previous question, what are my chances to conceive, as the past iui failures, ivf failure and cycle cancellations have depressed me to an extent. Any age factor to impact? Also would like to know your views on the other medicines which I am currently taking, as I mentioned in previous question. Also incase this FET gets failed as is expected, should I take a break as am on heavy medecines or should I done another FET in my next cycle itself? The reason fore to ask this is that my periods are very irregular and if I take break then maybe next 2 months will get wasted or maybe I will need to take 10 days do dusphaston to get periods, which I had to do in previous cycles. Kindly advice of the above queries. Thanks

Brief Answer:
Dont go for FET this cycle

Detailed Answer:
Hi,

These are oral medicines. Dosage and brand needs to be decided by doctor treating you after examining as it is not right for me to give prescription on that part. They do have side ffects like giddiness andhence need to be taken under monitoring by a doctor and dose to be adjusted.

Other medicines are good and you continue taking them. They are good enough. Take 10 days duphaston to get periods and then start the tablets I mentioned along with these.

Regards

Thanks a lot Doctor for your revert. It is surely very helpful. Basis all these medical details of mine, how much chances according to you do I have to conceive in near future with FET? Do you see positive chances for my endometrial thickness to improve by using prescribed medecines? Kindly share your views. I just wish for my endometrium thickness to grow in my next FET. Thanks. Kind Regards.

Brief Answer:
You have good chances of getting pregnant

Detailed Answer:
Hi,

I see there is possibility of improving endometrium in future and good chance of conception.

If endometrium does not improve, get hysteroscopy done to check uterine cavity.
