Question: I have a genital skin problem. I don't have pain or discharge from the lesions. My test results were negative for herpes. My doctor said if it was HPV she would know by the lesion characteristics. I have a little odor and vaginal discharge. The lesions are in a cluster on my left butt cheek. and a few up the sides of my labia. Please help

Brief Answer:
possible folliculitis....not herpes or molluscum variant

Detailed Answer:
Hello,
Thanks for the query to hcm ,

The lesions are definitely not herpes as they are not in confluence and not typical clear fluid filled vesicles with pain and itching. Also as the std tests are negative for the same, it further confirms it.
Another differential could have been Molluscum variant which is still a possibility but they are usually disc shaped with central umbilication and often accompanied with itching and have a self limiting course, painless but no scabbing.
Third differential is FOLLICULITIS which is more likely in your case as the lesions are separated from each other and related to each follicle with hair follicle seen coming out of it, inflammation present and if popped shall extrude sebaceous material . The likelihood of folliculitis is more in genital areas especially butt with excessive hair growth which tend to break while walking due to friction and get infected invariably, which is what has happened in your case.

Confirmation of local bacterial infection can be done by a local swab test and culture. Ideally oral antibiotics like penicillin group of drugs should be enough for a course of one week to take care of the infection. Along with it, regular local cleaning and hair clipping is advised to allow early healing. Keep the area dry and moisture free.
I hope i have answered your query in detail,
Wishing you good health,
Regards
