Brief Answer:
Probably urinary infection.

Detailed Answer:
Hi,
Welcome to health care magic,

Spasmodic pain and intermittent burning inside tip of penis could be due to variety of causes,

1) bladder infection is the commonest cause where the pain is referred to the tip of penis.

2) The other causes are urethritis or prostatitis,

3) Stones in the urinary tract might also cause pain over glans and tip of penis.

4) Application of irritant soap might cause pain as the glans penis is very sensitive

5) unhygienic conditions could also produce infection of glans.

In my advice, you should go for complete blood count,ESR,Urine routine micro and culture sensitivity testing to identify the organism and its sensitive antibiotics if you have not done it earlier.

Pelvic ultrasound with full bladder and swab test of the penis as per your doctor's advice and culture and swab tests must be done before starting of antibiotics.

Or if you have already started ciprofloxacin then it would have no use.

Quinolones groups (levofloxacin or ciprofloacin which you have been prescribed) would be a good choice in case of UTI and to be taken for full course like 7 to 10 days.

So,you should continue using ciprofloaxin as directed by your doctor with other symptommatic treatment.

Liquid citralka 2 TSF two times daily with water will be helpful for the relief of burning sensations.Or you may also use lignocaine gelly to be applied locally over tip of penis.

Maintain good hygiene of glans by washing with water and do not use soap to wash it.

Hope I have answered it completely and would be helpful to you.
