Question: I'm fifty years old. For the past few weeks I've been having high blood surges (up to 198/110) on three days. I was prescribed testosterone for Low-T about 3 months ago, which might have something do to with it. I was prescribed propranolol (10 mg) for my new high bp problem, in addition to the 20 mg of lisinopril that I already take. My blood pressure is now 109/60 (72 pulse) but I'm kind of light headed. I've been noticing increasingly that I get winded very easily (over the past 4-5 years, and worse the past few weeks or so). Any thoughts on what might be going on? I've had a recent electrocardiogram but never a stress test or test for heart blockages. Could this also be some kind of diabetic issue?

In the face of these bursts of high blood pressure, a second blood pressure-lowering agent should be prescribed. I would suggest that, if you take your lisinopril as a whole dosage, to separate it into two halves, which means taking 2 x 10 mg a day because it provides a better span of blood pressure figures control. I think that propranolol is a safe and effective agent to control the blood pressure, so I would advice you to continue taking it. The lightheaded episodes with approximately 110/60 mmHg of blood pressure are only temporary, until your body adjusts to these normal blood pressure figures, because this an organism that, until now, has lived with higher blood pressure figures and was adjusted to them.

Being a male of 50 years old, the risk factor being more than 45 years old, I would propose you to do further cardiac tests, just to exclude with higher certainty the possibility of coronary artery disease. Therefore, an echocardiogram would be appropriate, to evaluate the heart muscle, its chambers and valves by means of movement, function and size. Also, a cardiac stress test, to assess if there is a problem with a coronary artery or to exclude it. I would recommend, if you are a smoker, to quit it, and if you are a drinker, social, recreational or regular, to reduce it to 30 grams of ethanol a day (two beers or a glass of wine or a single shot of hard drinks such as vidka, gin or whiskey).
