Suggest Treatment For High Bilirubin Levels Along With Fatigue And Esophagitis

Question: hi, my name is XXXXXXX 20yrs old Male, had a routine checkup few days back ,the reports shows (Low average blood glucose ,Distracted CBC i.e having low Values of "monocyte ,lecuocytes,Basophills,MCHC"and High value of "MCV , RDW-SD,RDW-CV,PDW,MPV,PLCR") along with high bilirubin i.e 0.96 and SGPT i.e 59.53 and deficiency of 25-oh vitamin D i.e 13.23. my health history i m have Grade A - esophagitis and a small abscesses above the tonsil and acidity fatigue .
My question is what are the possible complications happening to me also what steps i have to take to eliminate the elevated values 1-the abscesses which i have is there above the tonsil with me since 3 months and had CBC done few times which shows no infection, so there is no possible complication by that it will be there with me as doctor said ,so infection is not due to abscesses.
2-acid reflux i m having i had been diagnosed with esophagitis since 3 yrs back and i took proper medications to eliminate and i guess it s eliminated by that.
3- how to kw the infections which i m having ? and what are the medications needed to eliminate the same ?
4-have distracted cough lyk white disturbed patches in water and have pain in the middle of left lung .
5- stress are upto high level :( in depression i m
6-i there any possibility that i m having HIV as leucocyte count is low , worried because i searched it on web the complications of low count ?
7- what if i take ofloaxain ordinazole for the same ?

Dear XXXX,

Better you told me the exact CBC values; have you any addiction?
I guess no.
You got megaloblastic anemia that indicates: liver problem, intestinal problem or lack of folate/vit-B12 like veg foods etc...
Acidity, esophagitis are related and told they are now controlled; tonsillar abscess indicated low immunity or local poor hygiene/trauma etc.
Bilirubin could be regarded normal if you are bit dehydrated or take less fluids...SGPT might indicate transient infection in pacreas, duodenum or biliary channels!
Deficiency of vitamin-D is common and don't think of kidney problem just looking at this!


Don't worry but you need some more work ups:

1. sputum examination / chest x-ray

2. blood immunoglobulin estimation

3. your bone marrow might be depressed; might require to see a hematologist

4. think of HIV only if you have contact history; there are so many mild/moderate immunodeficiencies also other than HIV!

5. THE DRUG COMBINATION you mentioned IS OKAY FOR tonsillar infection but not chest infections: chest infections better respond to azithromycin and amoxicillin...!

Don't take any unnecessary medicines or over-the-counter medicines..

Out of stress you over-interpreted your test results and I'm sorry that I missed the attachment!

See, all your biochemical tests could be regarded normal or just borderline or minor deviation that better to be evaluated by a clinician after a physical examination ; otherwise these are okay and random population pattern...

Your hemogram report shows some deviations but these are non-serious because the report is prepared by Fully automated bidirectional analyser (6 Part Differential SYSMEX XN-1000) and not rechecked manually! These minor variations are very common; please don't worry about this but repeat the hemogram from a local reputed lab and request for a manual recheck!

hi doctor in my report there is a deficiency of 25-OH VITAMIN D (TOTAL) hw can i rectify these low count ,is is same as Vitamin D only or other , what are the salt we can use to increase the level

TAKE VIT-D3 tablets only if you have problems

Well, dear XXXXXXX vit-D3 is a fat-soluble vitamin and in XXXXXXX population the lab reports show low values in many cases; therefore, if you have any problem relating to pone pains, joint pains, toothaches, neurological pains or back aches then only I suggest you to take Vitamin D3 /Calcium tablets but under an orthopedic doctor's surveillance; I mean you need some imaging of bones! Vitamin D3 enriched oils are also there like VANASPATI...!

I think it would be better to take simple nutritious foods for you rather than taking medicines at this age; go ahead, therefore, and revise your slightly abnormal reports they will become normal.
