Question: I have high blood presure taking medication does not seem to be working Amloaqapine lorstan
aturivistatin sterlyne and asprin. I have been taken of ramipril ,due to an attack of uticaria
had surgery for penile cancer i have cyst on one of my kidneys nail in leg bone from knee to ankle I have also had stents placed in heart artery Had dvt and perdicarditis pain in lower back also

2. how is your blood pressure, could you provide some data
3. could you write amlodipine and losartan doses
4. how long do you have swelling in your legs and ankles, have you ever been diagnosed with heart failure or renal failure
5. have you noticed any provocative factor for swelling

Hi pain constant dull get breathless when walking . Sit at my desk at work do not move around very much .Leg swelling goes away at night ,when I wake up it is gone. Blood pressure varies 151/93 196/128 194/120 152/92
Amlodapine 5mg Lorstan 50mg. Over weight I had kidney scan some years ago told I had cyst .or something in one. Never been diagnosed for heart failure or kidney failure.

Dear Sir,

1.Constant dull pain can be caused after pericarditis or there can be some joint problems
2. DVT can cause leg swelling, but they also can be as a side effect of amlodipin, and as a result of heart failure
If you were my patient I would perform

-echocardiography for evaluation of pericardium, and rule out cardiac cause for breathlessness and leg swelling
-lower extremities duplex ultrasound for ruling out venous cause for swelling

I would withdraw Amlodipine, increase the dose of Losartan and add diuretic

In this case there can be other options
1. change amlodipin to newer generation CCB, which have low probability to cause swelling of leg and increase the dose of Losartan
2. to correct potassium level and add diuretic which doesn't decrease potassium level, increase Losartan dose and try to withdraw Amlodipine
