Question: Hi,
please can you do some research to do a exhaustive list of the anaerobic glycolisis dysfunction caused by poisonings, diseases, nutritional deficiency, hormonal lack or excess etc. ?
I try to know what is my health problem.
You can consider that my only symptoms are, inability to do an effort that require the anaerbic glycolisis. Aerobic respiration seems to be good, because, I can run slowly for 45 min but I can't stand anaerobic work out like push ups, or a sprint. I feel like an accumulation of a waste or byproduct of glucose from anaerobic glycolisis in my muscle, during anaerobic work out. And I need to drink too much water to do this kind of work out, and with the big quantity of water it's a bit easier but still difficult.
I have this problem since 2010, now I'm 27.
Please just help me with doing the exhaustive list.

For the tests that I have done:

Seems to not have orthostatic intolerance
I eat a balanced diet.
My fasting blood sugar is in ranges.
Full blood count: normal, in ranges.
MCV: normal
ESR: normal
Cholesterol, triglyceride , HDL, LDL: normal, in ranges.
Liver enzymes tests: normal, in ranges.
Blood bilirubin: normal, in ranges.
Creatinine, blood level of iron magnesium, calcium, sodium potassium, chlore: in ranges
TSH and free T4 and free T3: normal, in ranges.
Tyroglobuline: : normal, in ranges.
Serum protein electrophoresis: normal, in ranges expect Albumine at 50,0g/L so 61,6%
C-reactive protein (CRP): normal, in ranges.
Prolactine: normal, in ranges.
Testosterone: 528 ng/dl so 18018 nmol/l : normal, in ranges.
Adrenocorticotropic hormone (ACTH): normal, in ranges.
antidiuretic hormone (ADH) : normal, in ranges.
HIGH Lactate dehydrogenase: 159UI/l so normal in ranges.
Lactic acid: 0,76mmol/l so normal, in ranges.
Vitamin: folic acid, thiamin 89 nmol/l, B6, B12: normal, in ranges.
My blood pressure is 120/80 mmHg when I see the doctor.
Sugar in urine, in ranges
CPK creatine kinase, in ranges

Brain MRI: found normal.

I have seen an ophthalmologist, he says he found nothing abnormal.
I have seen two psychiatrist they says they found no pathology.
I have seen a neurologist, he says he found nothing abnormal.

I have tried thiamine oral high dose and injections.
I have tried biotin 5mg a day during 1 month

Please help me with doing the exhaustive list.

Your Query
Hi, I'm a 27 years old man. Student in ecological solution. I do a little bit of sport. I live in France.
I have fatigue, and headaches when I do anaerobic exercice, and when I read a book.

Reading a book or something else give me headaches and fatigue, so I must to rest after reading something.
When I need to read something I must to drink a lot of water to be less tired after during less time, because If I do not drink a lot of water before reading I need to rest in my bed 5 time more.
I have stopped the school because of this problem.
and have done some test, standard blood test, tsh, lipid.
I eat a balanced diet.
I do one running of 1 hour by week.
And I want to do home bodybuilding like push ups, but I can't do it like I always did it because of this problem.

I don't know why my anaerobic capacity have decrease a lot the last 3 years.

When I do some push ups or an other anaerobic exercice like 100m speed running, I have really bad results, I take too much time to do this kind of exercice, I'm anormally fastly breathless and my heart beats are so fast, like I have never known that in my life.
I was not like that before 2011 but my physical appearance haven't changed.
I must drink a lot of water before doing sport because if I do not I can't exceed 10 min of sport.
I must drink 1 liter 35 minutes before to go running, If I don't I can't stand 10 min.
If I drink a lot of water like 1 liter 40 min before, I can do 70 push ups at the first serie, but If I drink only 1 or 2 glace of water 10 min before I can't do 45 and my heart begin to beat hard and I am completely breathless and tired.
After a running session of 40 min to 1 hour, immediately when I begin to do my legs muscles stretchings, even if I'm relaxed and feel good, feel good and completely not breathless at all, when I do my stretchings my heart begin to beat very fast and hard, and I begin to be breathless, then I must wait and drink water and do my stretchings very very slowly

Before 2011 I have never felt this, and I was used to work at school with pleasure read some books do sport more easily without this 1 liter before and 2 liters of water after sport.
My wear have a strange smell after sport, like I never smelled that before 2011.

I don't understand and the doctors here are too expensive.

Thank you if you help.

Doctor's Answer
Hello.I feel sorry for your situation.Gone through the history carefuly.

Needs really evaluation to find out what are you actually suffering with..especially in endocrinological, and cardiological aspects..

Seems to me,that you are suffering from an unusual condition or disease.

My suspect is you might be suffering with an hormone related disorder like atypical pheochromocytoma, carcinoid syndrome etc.

I would like to ask few more information to better comment...

1)What is your average blood pressure ? Do you get any BP variations ?
2)Do you excessively sweat ?
3)Any relevant family history like muscle dystrophy, hormonal disorders(pituitary, adrenal tumors etc) ?
4)Any hyperpigmentation areas on posteror aspect of neck, palmar crease region etc ?
5)Can you explain how the strange smell of your wear like ?
6)Any history of Constipation,or diarrhea,abdominal cramps near physical stress(workout) periods ?
7)Did you find any alteration in bladder habits ? like excess urination(polyuria) etc.A perfect urine intake and output chart would be better....

In my opinion you need definitely a local doctor's preferably an endocrinologist visit and evaluation.Reply me back with the above information to narrow down my suspects and further provide the help as much i can..Thank you..

Your Query
Hi, thank you for your answer.
1) My average blood pressure is 12.8 or 12.7 it's what I ear from doctors usually.
2) I don't really exessively sweat but I sweat a bit too much, I have never sweated too much 3 years ago. I sweat from under arms generally. But it's not too much exessive, because people don't notice it. I notice this because I sweated not like that before.
I sweat very exessively when I do sport running or football.
3) For family health issues, I have 1 brother in good health and two sisters in good health, my father and my mother seems to be in good health, I know that sisters of my mother has something like osteoporosis but they live in Algeria and they don't really eat a balanced diet because of a lack of money.
4) No hyperpigmentation areas on all the body, and palmar crease region seems normal with 3 lines and normal skin.
5) It's an unkwown smell for me, but something like a rotten wood plank. It's a smooth bad odor not too much agressive but really unusual.
6) No problem in this matter.
7) No problem in this matter. Normal for urine and bladder.

The few doctors I have seen don't know what is the source of the problem but we found that I don't have a good thermoregulation because I'm cold and need a lot of clothes. It seems like I feel better when I'm in vasodilation period like under the sun or in a hot place. So I tried a supplement called "citrulline" 6 month ago, to have more arginine in the body to have a better vasodilation. And indeed I feel really less tired since I take the citrulline. I feel like my muscle cells and other tissue are more supplied in something that I need in these cells.
Citrulline had helped a bit but it did not resolve the problem.

Thank you very much if you help.

Brief Answer:
Welcome to the world of hypochondriasis, where 1 check up is never enough.

Detailed Answer:
Dear Rawegan,

It's been a pleasure to go through the details of your case. And figuring your condition out has been a rewarding challenge for me. In medicine, when a patient has virtually all his biological levels at not just normal, rather perfect values, I'd say, and this patient visits multiple doctors for the same complaint, and this patient thereby get's nowhere despite all the effort;

It's time to look at your "illness" from a different angle altogether. I do not mean to hurt you or label you mentally ill. It is just a behavioural or thinking problem. Again I would like to put down my thoughts here and would not mean to ascertain my diagnosis.

As Clinicians, we strive to classify illnesses on 2 broad categories, like the day and the night, we have 2 broad groups of diseases that cause suffering.
We call this division, "Organic" and "Functional" illness. We say a disease is
organic, when there are definite, provable, replicable demonstrated pathological
changes to the tissues of the body.

You probably know that you've made an exhaustive list of all possible batteries of pathological and biochemical tests.

We are left with no option, when we find nothing physically wrong with your body, but to classify your disease as something "functional" ...

Something related to the the way your mind works... or doesn't work the way it
should, for you to lead a healthy, satisfying life.

We call this hypochondriasis, and since the mind exerts it's will over matter,
unresolved stress and emotional issues can actually make you physically sick.
Or at least... emotional issues can make you believe that you are really physically ill with manic intensity.

You've visited multiple Doctors, haven't you?
You've taken dozens of medical tests, right?
You've presented your reports in a neat and tidy fashion, haven't you?
And all this effort has proven nothing beyond a grain of sand that you aren't really vaguely sure that you have a disease at all.

In this situation we may be forced to admit that we may have a deficit of the organ of thought, the mind.

The brain is the most amazing machine in the world to instantly recognize patterns. This is coupled with the fact that the Brain is super efficient, so that
it achieves all it can with a minimal of resources. Thus the Brain is a creature
of habit. This was fine in human evolution when fine discernment in action was not required, in the primitive jungles.

And problems arise when desirable traits like discipline, method and organization, become stumbling blocks in life, because these patterns called
neural subroutines in neuroscience, take a life of their own and begin to dominate your life.

We call this OCD - Or obssessive compulsive disorder. Please read the book "Man Interrupted" over the Internet. You will probably feel a shock of recognition and instant identification with the hero of the book. His own biographer.

People have different obsessions and the type that deludes you is called as
a hypochondriacal obsession.

I would kindly recommend that you revisit a psychiatrist and you probably would like to ask for fluoxamine HCL. This is an SSRI agent that increases levels of a
monoaminergic amine in your brain called as Serotonin.

I'm telling you all this because you are very intelligent and like detailed answers.

The drug Fluoxamine was designed to eliminate OCD. You might want to ask your Psychiatrist for the Sustained Release Version that works best in your case.
Thus after 2-3 weeks of therapy, you'll "think" and "feel" that you are perfectly
fine.

Please be reassured. You're not crazy. You simply have a deficiency of Serotonin
in your brain, something like a vitamin deficiency of the body.

Take the appointment and take care.
