Suggest Treatment For Chronic Radiating Pain In The Spine

Posted on Fri, 24 Jun 2016
Question: I am 62 and had a vertical sleeve 2.5 years ago. 85% of my stomach is gone. Can't go back. Extreme arthritis has worsened due to muscle loss and loss of spinal support. I am on disability for my back, and have auto fusions in my neck, complicated by a reverse curvatue which continually transfers the pain up and down my spine. I am NOT suicicidal and regularly see a psychiatrist. I am controlling my pain with limited meds (Tylenol, NSAIDs, and klonopin for anxiety related to the extreme pain, narcotics, have, been taken off the shelf, which I was using in very limited amounts). I just went to the XXXXXXX Clinic which was a total waste of time. Did not consider my chief complaints, evaluation of possible treatments, or determining strateg to survive to a 100 years old (typical family longevity). My codition is worsened by perpetual nausea. Zofram is great but too expensive. I take Nexium am, Zantac pm and supplement with Tums, with little benefit. I am on too many other drugs which cause drowsiness, zofram works well. I can't take the NSAIDS which help but further aggravate my stomach. I would love to have the 85% of my stomach removed back. Pain is worse, can't take my NSAIDS, which I was assured I could prior to bariatric surgery. I use medicinal marijuana on a limited basis, but don't like the side effects. This precludes me from Narcotics which are only occasionally needed. Doubling my Nexium has been offered, but I already have a severe Mg deficiency. I am at a crossroads. I am NOT suicidial, but wake every morning wishing the Lord had taken me in my sleep. Zofram is wonderful, but not coveredby, insurance and cost prohibitive. I cannot tolerate meds which make me more drowsy since I already have a combination of those onboard. Are there alternatives to Zofram (non-fatiguing which could help with Nausea)? I am also trying to care for my wife who has M.S. Very limited faith in the medical community. This is not the way I treated my patients when I was still a practicing dentist. The paharacists and doctors don't seem to care, since their primary objective is to stay in good graces withe the DEA. Your thoughts would,be greatly appreciated. Currently scheduled, for, bilateral knee replacement and rotator cuff repair. Still trying to find a surgeon who will teat my neck and back. Very limited exercise helps, exceeding that causes me to be bedridden. I have ha numerous epidural so, facet injections, used TENS, hot whirlpools, bought a very high end adjustable mattress, use traction daily, along with physical therapy with little benefit. I can tolerate my mental and arthritic issues,,but the stomach pain is pushing me over the top. You suggestions would be greatly appreciated.

Sincerely, XXXXXXX XXXX

Brief Answer:
Ondansetron generics, metoclopramide etc.

Detailed Answer:
Hello and thanks for using HCM.

I have read your query and understand your concerns.

Sorry to hear about all these health issues.

Seems like you tried almost everything to deal with the pain caused by your spine condition.

In such stage, surgery should be considered (decompression of spinal cord, nerves and fusion to achieve spine stability).

This issue should be discussed with a spine Surgeon or a Neurosurgeon.

About Zofran (ondansetron), there are generic brands that may be used instead.

Other alternatives to lessen nausea (in place of zofran) can include things like Dramamine (meclizine, dimenhydrinate), like Benadryl or Zyrtec, metoclopramide (raglan) etc.

Since these are prescription drugs, you should discuss with your primary care Doctor about their use.

Hope you found the answer helpful.

Let me know if I can assist you further.

Greetings.

Are you aware of surgeries to section cervical auto fusions and orient in the correct manner? As a retired dentist and engineer this would seem to be the best alternative outside of becoming a quadriplegic due to undesirable effects. In the meantime I just transfer the pain up and down my spine Not treating root cause doesn't seem like a good long term strategy. Bariatric surgery was sold as improving my health, but it has only gotten much worse. Not correcting neck orientation will just eventually wear out opposing cervical vertebrae which are taking the additional load due to the reverse curvature. I assume the pain will just worsen. Final thought 1) Is sectioning and repositioning fusion an alternative a procedure you are aware of? 2) How high can a neuro-stimulator be placed (i.e. can it help with cervical pain at levels 3-5. Thanks for your input. Drs. I've seen either don't listen, care, or just want to push you off on someone else. Very hard to get questions answered. I would be willing to add bonus for helping me research. Haven't found good answers on my own. Sincerely, XXXXXXX XXXX,
