About two weeks I had experienced something like heart attack at night. feel acute pain and tightness to my left chest. Hard breath when laying down. and always felt lightheaded. The highest blood pressure test by an phone app is 171/105 when sitting and without exercise. average of one day is 151/95. And my heart rate was at average of 105 and highest was 110. but when i test with the blood pressure monitor at home it shows115/96. Is the monitor more accurate? Do i need to be worried for my blood pressure?

Detailed Answer:

Hello,

I passed carefully through your medical history and uploaded records&tests and would like to explain that;

- considering your blood pressure, heart rate values and especially your recent clinical symptomatology it is necessary to exclude/confirm a possible hypertension.

Smart phone applications are getting more and more accurate, nevertheless for properly addressing the issue of high blood pressure it is advisable utilizing a professional equipment such as a sphygmomanometer device or omron monitor.

In order to arrive to more precise average BP values, several blood pressure measurements should be recorded for a couple of days (several times daily) and refer them to your attending doctor for a clinical decision making.

You may also upload them here on HCM for a second professional opinion.

If your average BP values are close to the above high BP values referred by you, them is is advisable to go more in deep through a careful differential diagnosis work-up for properly addressing any potential causes of secondary hypertension.

In this regards, several tests would be necessary to explore;

- thyroid hormone levels,
- renal and liver function tests,
- blood electrolytes level,
- blood glucose levels.

Regarding your your LDL levels, I would explain that you are not at any increased cardio-vascular risk, because LDL/HDL ratio is less than 3, which indicates that you do not have dyslipidemia.

Anyway, if the presence of arterial hypertension is confirmed, I would recommend making some modifications to your diet, in order to provide lower LDL levels naturally. Aerobics can also help in this regard.

You should discuss with your doctor on the above issues.

Kind regards,

Dr. Iliri

I would recommend as follows:

Detailed Answer:
Hello again!

I would like to explain that single high blood pressure values are not indicative of arterial hypertension.

For this reason, I would recommend you to closely monitor your blood pressure values in the next week, frequently during the day ( in sitting position after 10 minutes of total relax).

It is important to write those values and show them to your doctor.

If high blood pressure values persist most of the time, starting antihypertensive therapy may be needed.

In the meantime, I recommend eating a balanced diet ( low fat and sugar and a lot of omega 3 fatty acids food), in order to help improve your LDL values naturally.

There is no need for stations for the moment.

Hope you will find this answer helpful!

Wishing all the best,

Dr.Iliri

The highest blood pressure test by an phone app is 171/105 when sitting without any exercise before. average of four day is 141/95. And my heart rate was at maxium of 135 average about 110 for four days. when i test with the blood test cuffs the hightese blood pressure is 185/149. average about 135/90 for two days. Is the monitor more accurate? I did more other kinds of blood test because i think the test i took was somehow inaccurate. I took the test around 2:00pm and I had fast for more than 17hours. will the result be affected? And these two days I found out I had black color stool... and i am not sure what they meant? is it normal?cause I've never had them before.

Brief Answer:
My opinion as follows:

Detailed Answer:
Hello again!

Thank you for the additional information!

Your blood pressure values seem to be above the normal ranges most of the time, which is indicative of arterial hypertension.

Blood pressure cuffs are more accurate for blood pressure monitoring compared to automatic machines.

Anyway, I would recommend discussing with your attending physician on the possibility of starting anti-hypertensive therapy.

It is also recommendable performing the above mentioned tests (thyroid hormone levels, cortisol and aldosterone plasma levels, kidney and liver function tests) to investigate for possible metabolic causes underlying high blood pressure.

Regarding the black color stool, they could be related to any special food or gastro-intestinal bleeding.

Are you taking any iron supplements lately?

If not, I would recommend performing a stool analysis for the presence of blood.

If blood is confirmed in your stool and you are not taking any iron supplements, I would recommend performing a fibrogastroscopy to check for gastric ulcers or gastritis.

You should discuss with your doctor on the above issues.

Hope you will find this answer helpful!

Kind regards,

Dr. Iliri

I would explain as follows:

Detailed Answer:
Hello again!

Thank you for the additional information!

I am glad to confirm that your hormonal tests are within the normal ranges.

It seems you have a urinary tract infection. For this reason, I would recommend performing a urine bacterial culture.

Regarding diabetes, I would recommend performing HbA1C levels, in order to completely exclude it.

I did not find thyroid hormone levels. Did you perform them?

Considering your high blood pressure values, starting anti-hypertensive therapy is necessary.

I would also recommend performing further tests to investigate for secondary causes of high blood pressure:

- cortisol and aldosterone plasma levels
- urinary metanephrines
- a Doppler ultrasound of the renal arteries or angio CT scan of these arteries.

Regarding your blood lipid profile, it is borderline and there is no need for any therapy actually. But, I would recommend following a balanced diet and performing a lot of physical activity.

You should discuss with your doctor on the above tests.

Wishing all the best,

Dr. Iliri
