Suggest Treatment For Bloating, Belching And Excessive Farting

Question: Hello Dr.,
I have been facing a problem of bloating, belching and excessive farting. This is more acute when empty stomach. So far my appetite is good and no weight loss is observed. I have been facing this problem since past two yrs. I visited physician and undergone endoscopy. He diagnosed my condition with hiatal hernia and suggested some medicines. Again I am facing this trouble which makes me uncomfortable. Pls advice.

I have read through your query in detail.
Hiatus hernia is a condition in which part of the stomach pushes itself above the diaphragm and into the chest cavity. This results in back flow of food and acid contents from the stomach into the lower end of food pipe causing heartburn. Initially medications and self care help a lot of patients. Over years it can progress to a larger hernia when a surgical treatment may be sought.

As you are having symptoms only since last two years, there is still a lot of time before any surgical treatment is required. You can continue with medications given by your doctor and making certain lifestyle changes.

Lifestyle changes recommended for you are:
1. Please take smaller meals throughout the day (every 3 - 4 hours) rather than a few large meals.
2. You may avoid foods that causes heartburn, such as onions, spicy foods, citrus fruits and tomato-based foods.
3. If you are taking alcohol regularly then please stop.
4. Eat at least two hours before you go to sleep.
5. Lose weight if you're overweight.
6. Please stop smoking.
7. Elevate the head of your bed 6 inches (about 15 centimeters).
8. Avoid bending and doing any work as this will cause discomfort.

Dear Sir,
Thanks a lot for your kind response. Further i would like to inform that, At present i do not have any heart burn problem. I am suffering from embarrasing flatulance and belching accomapnied with Disturbing stomach noises. I have attached few lab reports and scan reports for your reference. Also I apologise for not mentioning the correct term for diagnosis. As per report the condition is Reflux esophagitis. Also I have attached copy of prescription
Pls. Advice further..

Thanks for writing back with an update.

Relux oesophagitis is a much commoner occurrence than hiatus hernia. Medications are enough for treatment of reflux oseophagitis. The recommendations remain more or less the same even in relflux oesophagitis so please try to have small regular meals every 3 to 4 hours and avoid spicy food, alcohol and smoking.

Your ultrasound scan abdomen at that time showed a suspicious small intestine infection and it is most likely healed by now.

The TSH (thyroid stimulating hormone) and vitamin B 12 are normal.

Rablet is to decrease acid formation and may be taken under medical supervision.
Levazo is to make the food move forwards faster and prevent reflux into oesophagus.
Alprax is for anxiety and may be stopped.

To avoid passing gas please take small quantities of food regularly and avoid fasting for long hours. You may avoid taking gas producing vegetables like beans and cabbage.
