Suggest Treatment For Anemia When Diagnosed With Peptic Ulcer Disease

Posted on Tue, 29 Sep 2015
Question: Hello, I've been listed as anemic for the last 2 plus years. Iron deficiency so they said. I had a duodenal ulcer about 15 years ago. What makes me a little upset is that the gastro guy I went to two years ago and had both an endoscopy and colonoscopy with brushed off the results of the endoscopy as no biggy. I then found in the reports recently that after the biopsies I was listed as having Peptic Ulcer disease, and Erosive gastritis, erosive duodenitis, and erosive esophagitis. Yet no one can find any blood in my stool as in the first ulcer eruption. My hemo count hasn't reached over 10 in that time in with a transfusion of 3 units during the endoscopy. I'm at a loss. I'm just tired of feeling tired all the time.

Detailed Answer:
Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM). Iron deficiency anemia may be due to peptic ulcer disease or erosion in stomach and duodenum. If this is due to erosions only stool tests may be normal. The doctors have not prescribed you iron pills that is because iron pills may cause gastritis. However if after taking neksium still your hemoglobin is not improving then revisit your doctor for finding some other cause for iron deficiency anemia like malabsorption etc
I hope I have answered your query and this will help you. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.

Thank you. I truly didn't understand why no one provided iron pills nor why all stool tests came out normal. And trust me, I had lots of "digits" put in that place while I went through a battery of tests over the last bit of years. My PCP finally has a handle on the fact that I can tell him within a week if something he's prescribed, even BP medicine, will be intolerable. I just wish that hospitals would actually fill you with blood to above the anemic level so you can fight back a little bit. But I do know that trauma cases need that blood desperately and I would never want someone to bleed out just because I feel like dog crap. MY QUESTION IS: I can't possibly do an endoscopy again. Not happening. I fought the doctor and nurses under anesthesia and ended up in ICU after he gave me more anesthesia to calm me down, then he couldn't wake me up. So, QUESTION, how do I fix this because the guy wants to go down my throat again. I DON"T want an endoscopy. Some other alternative?????????

Brief Answer:
Alternative to endoscopy is capsule endoscopy

Detailed Answer:
Thanks for query. Other alternative of endoscopy examination will be capsule endoscopy in which you will have to swallow a capsule containing inbuilt camera which will take photographs of whole of the gastrointestinal tract and which finally comes out in stool without causing any discomfort. Other advantage will be that in endoscopy and colonoscopy which you have underwent examine only esophagus, stomach and very small part of small intestine and whole of the large intestine respectively. And the capsule endoscopy will also examine whole of the small intestine which is left till now.If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.
