Suggest Treatment For Alcoholic Liver Cirrhosis And Pain In Stomach

Question: My father has alcoholic liver cirrhosis he has ascites and now started feeling tired and pains in stomach with short of breath. We have done endoscopy recently with banding.
I want to know which is good Lasix or dytor 20 mg ?

I am Dr.R.K and I am pleased to assist you.

Both Lasix and Dytor are equally good. But Dytor has a longer duration of action.
Lasix and aldactone both should be taken for effective urination and improvement of ascites.
But the most important thing to do is salt restriction. Salt intake should be less than 2 g/day.
If the response is inadequate then the dose of Lasix and aldactone should be increased..
Sometimes even after giving the maximum permissible dose ascites would be still troublesome and it is called diuretic resistant ascites. In this condition other treatment options should be followed which includes large volume paracentesis, albumin infusion and TIPS.

1.What is the alternate solution.Can you please study the reports and let me know what all options do we have to treate this or atleast stop further damage?
2.looking at the age and other parameters in reports and clinical notes would you recomend liver transplant ?

Hi,
As said earlier he is probably having diuretic resistant ascites ( I presume salt restricted diet is given to him).
The options available are large volume paracentesis, albumin infusion,TIPS and Liver transplantation.
But taking his age in to consideration I would not advise a transplant. Moreover for transplant he has to be registered in organ transplant registry and the priority is based on certain scoring system.
