Posted on Wed, 22 Jun 2016
Question: I am very groggy after I eat and can last for hours. I work and it been hard. I had some lab work done. My AIC was 6.5 but my glucose was only 90 in am. My Vit D was 17L. My B12 was 1338H. My thyroid Stim Hormone was 5.17H but my Free T4 was normal. I had more test but no results yet. I have been on Vitamin D 50,000 and Synthroid 88mcg for 2 1/2 months now. My present Dr. says I am technical a Diabetic but I haven't been put on anything. You will see in y history that I had Insulinoma when I was young. My blood sugars have bounced back and forth from high to low for years. I am sure stress has played a part in it. I was at UT, in XXXXXXX summer of 2014 because I was having so many problems with my BS?. I was taking some nature product and by the time I got admitted I felt better. I also passed their test. I was put on Metformin in 2013 to try and figure what going on. I felt better but then 2014 I had to stop the drub because it seem to make things worst. I had a small part of my pancreas removed in 1966 and was toldI might become a Diabetic when I was older. It seems I have been trying for years. Can you make any sense out of this information? I started back on Metformin ER 1/2 500mg BID that I was on before today out of desperation. I don't see the Dr. until Monday about my new lab results.

Brief Answer:
Diabetes

Detailed Answer:
I follow what you are saying here.

You seem to have developed type 2 diabetes. With these type of blood glucose levels, endocrinologists tend to offer only lifestyle modification as a first treatment option.
So essentially, healthful eating and increased physical activity should help keep this in check.
If you are overweight, losing 5 to 7% of body weight will also be immensely useful in reversing this problem.

When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH
Free T4
Anti Thyroid Peroxidase (TPO) antibodies
Anti Thyroglobulin antibodies 12 hour Fasting Lipid profile
Urine albumin to Creatinine Ratio

None of these tests require any fasting and can be done at any time of the day
