default
Posted on Thu, 20 Feb 2020
Question: I have a tightness/dry/stiff feeling in the back of my throat. I also have lot of dry cough for the last 3 weeks.
Last week I had a feeling of something stuck in my throat and every now and then have a feeling of mucus stuck in my throat.

This all started after a bout of fever in the 1st week of XXXXXXX wherein the fever and cold went away. This was followed by 1 week of dry mouth and throat and dry cough.
Last week I had a bad bout of gerd and since then my throat(back of throat) feels irritated and when I cough I don’t feel my throat. I sometimes feel the air gets stuck in the throat when I am trying to breathe.

I have tried drinking hot water, salt water gargle, cough syrup, steam nothing ha helped. My throat still remains the same.

I am really worried about the condition.
Could this be a result of anxiety and gerd. Today I had some mild hoarsenss and have a feeling of my throat is churning

Brief Answer:
Laryngopharyngeal reflux

Detailed Answer:
Hi

Thanks for posting the query

I have gone through your query and understood your problem. You seem to be suffering from Laryngopharyngeal reflux secondary to gastroesophageal reflux disease (GERD).

1. The symptoms have got worsened after a bout of viral infection due to the stress following the infection.

2. The throat symptoms due to GERD is common nowadays. This is due to the reflux of acid from the stomach into the throat causing inflammation of the mucosa of the throat. The inflammation thus caused gives the feeling of a foreign body stuck in the throat.

Secondly, to reduce the inflammation lots of salivae are produced, which gets accumulated over the inflamed area. When the acid trickles into the vocal cord area you may get cough and choking sensation.

3. My suggestions to you are:
a. Drink frequent sips of warm water
b. Take frequent small meals. And, a light dinner.
c. Give a gap of at least one hour before you hit the bed following the dinner.
d. The head-end of the bed should be slightly elevated in order to prevent the reflux of acid from getting into the throat.
e. You would require proton pump inhibitors and prokinetics to prevent acid reflux.
f. Avoid excess coffee/tea/spicy/oily food/smoking/salt water gargling.
g. Avoid stress, this can worsen your symptoms.

If the symptoms do not improve please consult your doctor for further evaluation and follow-up.

Hope this answers your query. I will be available for the follow-up queries.

Thanks for answering my question dr.

Why is my throat not getting better, even when I cough I sometimes don’t feel my throat?

I have tried drinking hot water, gargle with hot water, steam. It’s just not getting better.

How long in general the throat takes to get back to normalcy? I am trying to get an appointment with an ENT, can the ENT see any muscle weakness or irritation or anything wrong even neurological symptoms such as atrophy?

I have been under massive anxiety in terms of ALS for the last 3-4 weeks and then this thing came along, I feel the more anxious I get the dryer my throat and mouth gets.

Also back of the throat feels like it’s filled with mucus or stiff. When j swallow my mucus either feels like a lump or filled with mucus.

Can an ENT let me know what’s wrong if there is any? I am pretty sure it’s not globus

Brief Answer:
Throat discomfort is due to the stomach acid burning the mucosa

Detailed Answer:
Hi XXXXXXX

Thanks for writing back

My sincere apologies for the delayed response. First of all, I would like to reassure you that the throat discomfort what your perceiving is due to the acid reflux. This has got worsened because of your anxiety. Unless you don’t overcome the anxiety trust me, you would not get better.

1. Drinking hot water and doing gargles with hot water is going to worsen your symptoms. This is because, the hot water when it reaches the inflamed throat mucosa, the inflammation gets worsened and enhances your problem. Hence you need to drink frequent sips of lukewarm water and not hot water. Lukewarm water will increase the blood circulation to the already inflamed mucosa of the throat and helps in healing faster.

2. The cough is due to the presence of inflammation in the throat. When your tongue touches the back of the throat and rubs over the inflamed area the cough is produced. Unless the inflammation doesn’t come down the cough is not going to get relieved.

3. Anxiety is going to cause the spasm of the involuntary muscles in the neck; this is going to add on to your problems. Hence, I would request you to reduce the stress and anxiety.

4. Again, the sense of food sticking into the throat is due to the dried up mucosa following the throat inflammation. Drinking frequent sips of warm water is going to help in relieving this problem.

This condition is usually noticed in people having recurrent nasal block, mouth breathing, acid reflux disease and taking certain medications like antihistamine.

5. The sense of choking is due to falling of the stomach acid onto the vocal cords. This usually happens when you lie down. Hence, you would require medicines like prokinetics to increase the gastric emptying.

6. I doubt any neuromuscular problems in your case.

7. You would require medicines to relieve your anxiety. Anxiolytics can help in reducing your stress and calming your brain. Consult your doctor for the prescription.

8. The globus sensation is due to the anxiety and acid reflux as well. Unless the causative factors are not taken care of, your symptoms are not going to get relieved.

9. I would encourage you to do meditation for at least 10 minutes everyday. This would keep you relaxed. Go for a walk, watch some comedy movies or TV soaps.

10. Consult your ENT doctor for a thorough evaluation of the larynx and pharynx. Once, the medications such as proton pump inhibitors are started it would take a while for the symptoms to get alleviated. The medications should be take for a minimum period of 2-3 months.

Hope this clarifies your doubts. Revert back to me if you have any questions.

Ok, I have read that the feeling of something sticking to the back of the throat is a sign of dysphagia?

I also have a constant scratchy throat and it’s interfering with my speech like it feels really irritated when I am talking and sometimes I have to cough or clear my throat before I talk.

Is this due to gerd plus my anxiety causing the symptoms to be felt more?

Also the sticking sensation as I have read is it dysphagia?
When is the sticking to the throat feeling is a sign of dysphagia?

Brief Answer:
You do not require any Esophageal manometry at this stage

Detailed Answer:
Hi XXXXXXX

Welcome back

1. feeling of something sticking to the back of the throat is a sign of dysphagia?
A: No it is not dysphagia. Difficulty in swallowing either liquids of solids is known as dysphagia. Sticking sensation is not dysphagia.

2. have a constant scratchy throat and it’s interfering with my speech
A: This is due to spilling of the regurgitated acid contents onto the vocal cords. Hence, to prevent this we need to take prokinetics.

3. Is this due to gerd plus my anxiety causing the symptoms to be felt more?
A: Yes, you are right. Laryngo-pharyngeal reflux is due to acid reflux disease. Anxiety and stress worsens this condition.

4. should this in itself rule out dysphagia or should I still go ahead and get a swallow study or endoscopy done?
A: That is not required at present. Take the medications judiciously and follow the lifestyle modifications. You will definitely get better. The swallowing test is required when you have actual difficulty in swallowing.

5. after eating yogurt my throat was kind of burning, and had to double swallow like some of the yogurt I had to bring bang to the mouth and then swallow is that normal?
A: I think you are worrying unnecessarily and constantly thinking about this issue. Stop worrying and keep yourself relaxed. Start taking the medications as described in the earlier response.

6. Also what’s causing hoarseness i. My voice even though mild. I feel it’s by raspy
A: Due to spilling of the regurgitated acid over the vocal cords your voice gets changed or strained.

7. some pieces of food seem to be sticking and some don’t
A: That is chiefly due to the irritation of the throat mucosa by certain food materials.

8. Do you think this is sign of dysphagia?
A: No, this is not a sign of dysphagia.

9. Do I need to get a swallow study done?
A: I don't recommend the study at this point of time. Let us wait for few more days. Try taking proton pump inhibitors, prokinetics and follow the lifestyle modifcations. If this fails, only then we can switch to the next step.

Hope this clarifies your doubts. Wish you good health.

So what exactly is dysphagia, isn’t it like difficulty in swallowing?

I feel like it’s getting blocked and even after eating food feels like something’s there. My throat is sore and scratchy throughout the day. Even when I drink liquids I feel something in my throat but no problems with swallowing as such.

Do you still think this is not dysphagia and I don’t need any follow up tests?

My doctor has given me Pepcid 20mg and Prevacid 30mg and asked me to eat it at the same time, like a high dose

You are having foreign body sensation in the throat and not dysphagia

Detailed Answer:
Hi XXXXXXX

Welcome back

Patients with dysphagia take more time and effort to swallow the food into the throat and later into the food pipe. Many a time, it may be associated with pain. In your case, its the sensation of something stuck (foreign body sensation) in the throat.

Your doctor has prescribed the right medications, with the appropriate dosage. But, a prokinetic should be added to this to increase the gastric emptying and tighten the lower esophageal sphincter.

Please keep yourself relaxed, without which the medicines will be futile.

I hope this clarifies your doubts; wish you a speedy recovery.

I too take little more than usual in terms of time to swallow my food. Like I was eating today and I for sure knew the food was staying bit longer than usual to go past the throat, I had to bring them up and then swallow again.

1. I am not sure if this is due to gerd or what, I am pretty sure it’s not a sensation any more I am having mild difficulty In swallowing. Could be due to gerd/lpr?

2. Is this an indication of dysphagia what I described, like food staying in my throat and then bringing it back up and then swallowing them?

I am praying it’s not due to any neurological issues.

3. I have one more question,
So when I go and see an ent and if the issue is still there, is it okay if I eat like 20-30 mins before my appointment as I still have the feeling of something in my throat or an irritated throat feeling?

4. Can dysphagia cause throat irritation or soreness?

For me swallowing is not an issue no pain, coughing or drooling or anything like that.
My main issue is the food sometimes stays on the throat and then I have to bring it up or swallow 2-3 times to get it into the food pipe.

Some foods I eat there’s little to no problem. But I still get an irritated throat. Like I eat a cucumber raw one, I got this scratchy feeling while eating, like it’s scratching my throat and causing soreness.

Any food that has a thick consistency gives me this issue of being stuck on the throat, not all of them but in the middle of my eating.

Do you still think that I should ask for a barium swallow?
I got my endoscopy done last XXXXXXX and ph and manometry study done in July.

So feeling of mucus in throat is a classical example of LpR?

The findings of endoscopy was all normal nothing that would be reportable.

Also, you are saying I am having perceived dysphagia?
I had a normal swallow today but always get this scratchy and sore feeling after eating a meal and some dry coughing.

My dry cough is due to Lpr?

Thanks for answering all my questions.

Also what home remedies I can take to get rid of this problem?

And how common is this complaint amongst patients?

Also with dysphagia it won’t improve right like one day will be good the other day won’t be?

With neurological conditions difficulty swallowing would be present for both liquids and solids right?

Could my issues be due to inflammation due to lpr? Can an ent spot the inflammation or something which would explain my situation?

My main concerns are something mucus or something in my throat, dry cough and sometimes difficulty swallowing

1. But doctor with globus you feel like a lump or something right? I dont feel like that, my throat feels sore or dry and also when I cough I sometimes dont feel my throat.

Yes, in the morning my throat is sore and itchy and also sometimes in the mucus i get yellow color in mucus otherwise its white.

2. So you did say you saw 1 neurological patient in your career, can I ask you what disease or condition they had and what were their symptoms and how long did they have it? How old was the patient?

3. Do patients my age come with this complaint, I don't smoke, but in a year like I do hookah 2-3 times? I have had issues with gerd for almost 6 years now and last year was like severe gerd. Continuous belching, some heartburn, pain in the sternum and sore throat.

4. So this throat thing could last for some time, right? So when should I worried about the throat sensation? Like sudden sever dysphagia or loss of voice or speech issue? Thats when I should be concerned?

I am also getting sinus issues now I feel thats due to anxiety and gerd. Not sure why, can you please answer my last bit of questions, you have been very patient in answering all my questions. Thank you for that.

I have read a case of a MND patient who had a similar thing, but he had different things going on at the same time, like continuous twitching, intense cramping. Then in like 3-4 months he developed excessive mucus in his throat which could not let him eat without constantly clearing his throat and then in like 2 months he sort noticed his voice going and in the next 2 months he started slurring. Plus he also had some atrophy in his hand from the very beginning. He will righting his case also said this that his symptoms should not be selected individually but rather as a whole which would tell the story. He was a 50-51 year old white male.

I am 29 year old asian male with no history of any familial cancer or MND or anything bad. I have GERD and diabetes running in my family.

I know my symptoms are very likely not due to anything bad but a combination of lpr and anxiety but all these issues have come in when I started twitching like 6 weeks ago ( twitching has gone down by 90-95% ), my random sensations like numbness, stiffness even though there have also gone down. The only thing thats there and not going away is this throat thing. All this started after I got a viral fever in the start of the year and I was coughing like mad guy and had fever for 2-3 days. The cold went away, but the dry cough stayed and had bad anxiety due to MND fears and then got dry mouth and throat and the dry cough remained.

The the dry mouth went away in like 2 weeks, but now i have this feeling of something in throat or a dry throat, the dry cough is still there but again even thats reduced. My throats scratchy and irritated and food irritates it bit more.

That's the story of my last 6 weeks. I did have a clean emg and physical examination done by a neurologist. He did not check my bulbar muscles though, he told me he doesn't see any signs for it and plus the emg on my limbs were all textbook normal.

I still twitch, but have been told twitching is normal and probably lot of people twitch its not people just don't pay attention to it. Plus anxiety makes things worse.

I dont twitch when I am outside or busy its only when I am by myself and like feeling really anxious I get a really bad dry mouth and twitching episodes.

I just pray like this thing gets better in the next 2-3 weeks. So this is the 3rd week of the throat issues and 2 weeks since something in throat feeling. Have felt better/normal for a day or 2 but then something else happened and anxiety creeps back in and so does the problem.

I have a question, is there a permanent treatment for lpr or is it like lifelong management?

What are my chances of oropharyngeal cancer? And esophageal cancer? Since I seem to have a bad case of GERD/LPR.

Also, why am I feeling this sensation? Is this like a spasm or an inflammation of the throat area? Or is it something else?

Should I try and stop to swallow more mucus to get rid of this issue?

Should I stop trying to spit out the mucus because I am continuously working my throat in a bid to get rid of the problem?

Am I making things worse by doing the above and should I control the urge to cough and clear my throat? I sometimes do it just to feel and try to feel normal.

To summarize:

1. But doctor with globus you feel like a lump or something right? I don't feel like that, my throat feels sore or dry and also when I cough I sometimes don't feel my throat.

I am also getting sinus issues now I feel that's due to anxiety and gerd. Not sure why.

5. When I am eating something like a consistency of a sticky rice or sticky pudding or something which is bit sticky I find it kind of sticks to my throat and I have to put little extra effort to get it down, but it does not block my swallowing or choking or anything like that, that extra effort irritates my throat even more.

Is this normal in LPR, and should I do something to get rid of this thing? In general how long does it take for me to see some improvement with the condition, not likely feel 100% normal but atleast 10-15% improvement?

6. What are my chances of oropharyngeal cancer? And esophageal cancer? Is there a permanent treatment for lpr or is it like lifelong management?

11. Do people come in with fears of MND? Or is it that I have read stories and my mind if fixated on this condition even though I might not have anything as bad as that?

12. Do people frequently visit like repeated visits that the throat is not getting better even after months of the sensation in the throat and perceived dysphagia? How do you treat or handle such patients?

Since I seem to have a bad case of GERD/LPR.

7. Also, why am I feeling this sensation? Is this like a spasm or an inflammation of the throat area? Or is it something else?

8. Should I try and stop to swallow more mucus to get rid of this issue?

9. Should I stop trying to spit out the mucus because I am continuously working my throat in a bid to get rid of the problem?

10. Am I making things worse by doing the above and should I control the urge to cough and clear my throat? I sometimes do it just to feel and try to feel normal.

The only thing that's there and not going away is this throat thing.

Can you please answer my last bit of questions, you have been very patient in answering all my questions. Thank you for that.

15. Also while speaking I feel stiffness/dry throat and some dryness in nasal passage and I feel due to that the speech sometimes gets affected like instead of dry I end up saying “darrayy” or throat ends up as “tharoatt” ,this is just one word but there are other words too. But then when I try to repeat the same words it comes out alright. Not always but sometimes, Is this due to anxiety or could be something else?

Also while taking my throat feels irritated, so all these issues together are making me really anxious and worried.

I have lost like 2 kgs, this month like I have been drinking a 1-2 liters of water extra and also have some diarrhea due to anxiety for 2 weeks, could this be the reason for the 2 kgs lost? I have lost some fat/inch loss throughout the body. My shirts and trousers were but tight now they feel lose.

So all these things together have made me worried.

The feeling today is more like throat tightness or pressure , what could be causing that? Swallow was normal today apart from few grains in the mouth feeling

Dr one more question how can I rule out that my throat issue is not due to MND?

Is this throat tightness and feeling of something very common with Lpr?

Brief Answer:
Your anxiety is the prime cause of all your problems...

Detailed Answer:
Hi XXXXXXX

Thanks for writing back

1. But doctor with globus you feel like a lump or something right? I don't feel like that, my throat feels sore or dry and also when I cough I sometimes don't feel my throat.
A: Yes, globus is foreign body sensation in the throat. Soreness and dryness is due to constant reflux of acid in the throat.

2. So you did say you saw 1 neurological patient in your career, can I ask you what disease or condition they had and what were their symptoms and how long did they have it? How old was the patient?
A: This patient I saw was 13 years back. A 70 year individual had a stroke. The stroke was managed by the neurologist. The patient did recover partially.

3. Do patients my age come with this complaint, I don't smoke, but in a year like I do hookah 2-3 times? I have had issues with gerd for almost 6 years now and last year was like severe gerd. Continuous belching, some heartburn, pain in the sternum and sore throat.
A: This is a common problem nowadays due to the change in lifestyle. You said it all. Your GERD is the prime cause of all your problems.

4. So this throat thing could last for some time, right? So when should I worried about the throat sensation? Like sudden sever dysphagia or loss of voice or speech issue? That's when I should be concerned?
A: Yes, it would last for sometime. I would appreciate if you could wait with some patience for few weeks. You would definitely do well. Secondly, you need to take the medications regularly.

A: This sensation is due to the presence of granulation in the mucosa of the throat following LPR. You need to take the medications regularly to get better. Also, you need to change your lifestyle.

6. What are my chances of oropharyngeal cancer? And esophageal cancer? Is there a permanent treatment for lpr or is it like lifelong management?
Since I seem to have a bad case of GERD/LPR.
A: Seldom, the untreated cases may develop cancer. Yes, you need to take the medications for a longer duration.

7. Also, why am I feeling this sensation? Is this like a spasm or an inflammation of the throat area? Or is it something else?
A: This is a spasm in the smooth muscles below the mucosa. Also, the inflamed mucosa can give rise to foreign body sensation in the neck.

8. Should I try and stop to swallow more mucus to get rid of this issue?
A: Yes, you should try to swallow the secretions. Theses secretions are produced to soothe the inflammation in the throat.

9. Should I stop trying to spit out the mucus because I am continuously working my throat in a bid to get rid of the problem?
A: Spitting the secretions is going to worsen your problems and later, it becomes habitual for you.

10. Am I making things worse by doing the above and should I control the urge to cough and clear my throat? I sometimes do it just to feel and try to feel normal.
A: Yes, obviously.

11. Do people come in with fears of MND? Or is it that I have read stories and my mind if fixated on this condition even though I might not have anything as bad as that?
A: The prime reason behind this is anxiety. You need to contain that as soon as possible.

12. Do people frequently visit like repeated visits that the throat is not getting better even after months of the sensation in the throat and perceived dysphagia? How do you treat or handle such patients?
A: Yes, they do visit us back when they are not getting better even with medications. Then we do counselling for the patient and change the medications if necessary.

13. I just pray like this thing gets better in the next 2-3 weeks. So this is the 3rd week of the throat issues and 2 weeks since something in throat feeling. Have felt better/normal for a day or 2 but then something else happened and anxiety creeps back in and so does the problem.
A: As I mentioned in my earlier response, the anxiety is the prime cause of all your problems. You need to get it sorted out. No amount of medicines are going to be effective for you unless your anxiety is under control.

14. Not always but sometimes, Is this due to anxiety or could be something else?
A: That's because of your dry mouth which causes the sticky tongue and the some resonant words may be missed. You need sip few drops of water and then continue talking.

15. Also, what’s causing dryness in roof of my mouth? Is this due to Lpr or just anxiety?
A: Both the above conditions can cause dryness in the mouth

16. Could this be the reason for the 2 kgs lost? I have lost some fat/inch loss throughout the body. My shirts and trousers were but tight now they feel lose.
A: Your anxiety is the prime cause of all your problems.

17. The feeling today is more like throat tightness or pressure , what could be causing that? Swallow was normal today apart from few grains in the mouth feeling
A: When a person is sad or nervous the muscles in the neck go for involuntary spasm. Keep yourself relaxed. Watch few comedy shows or movies. Go out with friends/relatives and chill out.

18. Dr one more question how can I rule out that my throat issue is not due to MND? Is this throat tightness and feeling of something very common with Lpr?
A: I have answered this question above. Once, you start keeping cool, this will become much better. The sooner you start getting yourself relaxed, the faster is your recovery.

Hope you are satiisfied with the above responses. Wish you good health.

I Have A Tightness/dry/stiff Feeling In The Back Of My

I have a tightness/dry/stiff feeling in the back of my throat.
