Question: how to increase blood platelet when its very low 20000 or less. hemohlobin is also very low 6.4. doctor tried put 16 bottel platlet and 12 bottle blood.

let me know how yo make patient better.

Based on your query, my opinion is as follows:
1. Single donor platelet transfusion, every two days, three times should be helpful. Each exchange will increase by 10-15000, leading to 50-60000 count within a week. Pooled donor platelets can cause immune reactions due to multiple antibodies, which can cause platelet refractoriness.
2. Still better would be HLA matched platelets.
3. RBC packed cells can be given. For 6.4g%, unless there is any complications, no further transfusion necessary.
4. Nutritional supplements with b12 and folic acid is necessary.
5. Need to watch for any internal bleeding. When the count falls less than 10000, risk increases of internal bleeding.

yes the patient have upper GI bleeding and doctor don't want ti operates until platelets count reach to 50k or more. let me know how should approach to tell current doctors. do you know any good doctor or hospital in XXXXXXX which can treat this sitution.

Risk of bleeding increases during invasive investigation.
Improvement can be done through evaluation of immune antibodies via lymphocytotoxicity test or platelet antibodies evaluation or even platelet crossmatching. Most of the setup with dedicated transplant unit or transfusion medicine department in all major setups should have this.

If not platelet apheresis from single donor can be used to raise platelet count. Single donor platelet can be collected up to 3 times in a week.

Discuss with your doctor regarding single donor platelet or immune antibodies evaluation.
