Does Having Unprotected Sex Automatically Lead To HIV?

default
Posted on Fri, 4 Sep 2015
Question: On 9-8-15 I went to have a full body oil massage. So for 25 mins the female gave me massage then asked to provide oral sex. For another 10 mins she gave me handjob and oral sex.. I couldn't ejaculate so I asked whether I could hv intercourse with her.. she said yes.. I'm inexperienced so I forgot about the condom.. n told her to guide me where to enter.. I kept thrusting for 20-25 seconds.. didnt feel any pleasure and as time was up n I still couldn't ejaculate so I dried myself off with a towel washed my genital n reached home again washed my genital.

Later I read about unprotected sex.. n I hv been since reading about std's n Hiv.. im utterly scared and since then I'm checking for any kind of rash or fever or fatigue.. today's 2nd day I cant stop thinking about it.. felt fever when waking up n fatigue when two times today.. im preoccupied by the hiv infection. N before the massage since 26 July till today I woke up every day with cold and running nose.

I have no rashes.. I'm 25, a telecom engineer. Male. Thus was unintentionally first time I had unprotected sex. I had sex twice only before this, protected.

I'm immensely scared as I might get infected. Please forgive me for my lengthy message.. please help me. Plus I don't go to parents n my niece bcoz I don't want to infect them if I hv any contagious disease. I'll visit a doctor tomorrow. But i need your help too. Im very scared. Im the only boy and I don't want my parents to suffer because of my mistake.

Please help.

Brief Answer:
Please visit HIV center now and get tested after one month

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. The risk of HIV from the above incident is 1 in 2000 cases. It is too early to confirm HIV infection due to the unprotected sexual intercourse.

2. You might consider visiting a HIV counseling center at the earliest and start taking anti retroviral post exposure HIV prophylaxis which is a course of medicine for prevention of HIV. For this you have to consult the doctors at the HIV center within 72 hours from the time of probable sexual exposure and discuss your incident in detail. If 72 hours are over then it is not possible to take post exposure prophylaxis.

3. Please get 4th generation rapid HIV screening test done after a month from date of unprotected sexual intercourse. This is a 4th generation rapid test called HIV 1 and 2 antibodies and p24 antigen combo test. The results are accurate after one month.

Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Thankyou so much Dr. XXXXXXX
My district has an ART center. I'll visit it tomorrow. Get myself treated. The female massager though told me that intercourse wasn't allowed and she did this because she found me nice n told me to keep it secret. But i still don't know her health history and this was third time I had intercourse in my life.. thankyou so much for the guidance.. I'll leave a message after I get the meds tomorrow.
Forever in your gratitude.
Take care Sir.

Brief Answer:
You are having a common cold and flu most likely

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

1. Yes, now I can see both the screen shots. Thanks for your prompt update.

2. In the first picture, you have written about getting tested for VDRL, HIV, Hepatitis B and the reports are negative. It is early to get tested for the infections and I recommend getting tested in the following way for peace of mind
(i) VDRL after 30 days
(ii) HIV by 4th generation rapid HIV 1 and 2 and p24 combo test at 30 days
(iii) Hepatitis B at 6 months

When these are done as recommended then it is accurate. I hope your reports will be negative and please do not panic.

3. It is better to forget about taking prophylaxis because 72 hours are over. Therefore thinking more about it is not of much help.

4. Flu symptoms will never happen 2 - 3 days after infection by HIV. This is probably common cold. It might happen due to weather changes or after stress from not getting sleep and thinking too much. Please relax your mind. Take medicines and do warm water gargle twice a day for a week. You will feel better.

5. The problem with masseuse is that they might always tell lies. I am not trying to scare you, probably she is clean but there is no need to go and talk to her again. I assure you that HIV does not get transmitted easily and therefore the chances are only 1 in 4000 that you might be infected. This is really a small chance.

Anyone can make a wrong judgement and you have unknowingly went to have sex without condom. Please do not worry and I have come across many patients who are in your situation and most of them test negative after 1 - 6 months for HIV. You please ask whatever doubts you have in mind and I will answer back in detail. Do not get worried, you are a good person and will have a long and healthy life.

Regards,
Dr.Vivek
