Posted on Thu, 26 Feb 2015
Question: Can continued use of clonazepam, tamsulosin, baclofen, gabapentin, morphine sulf, along with large injections of didlaudid, valium, torodel and a morphine pump with a catheter not attached to the spine and severe pain and stress cause heart damage such as inferior infarction, anterior ischemia and anterior T wave changes and result in death. All of this over a period of approx 4 years.

Depends on the health issues the patient is having...

Detailed Answer:
Hi,

I have gone through your question carefully and understood your concerns.

For your information, infarction is caused due to blockage of coronary arteries and due to atherosclerosis.

I have gone through the reports you provided and I could not find any reports regarding his cardio-vascular system. I suppose he was not having any problem that there was nothing noted.

Hope it was of help! Let me know if I can be of further help!

Thank you. What is and causes Ischemia. He did not have high blood pressure and no indications of any kind of heart problems. The only way I know abt the heart is from the ECG's taken in the ER, which I did not receive until after he passed away. We were never told. He always thought he had indigestion and would eat Tums. So just to be clear, you are telling me that severe pain, narcotics and stress does not have any effect on the heart?

Detailed Answer:
Hi back,

Thank you for following up.

Myocardial infarction is caused by narrowing of heart blood vessels due to atherosclerosis.

There are many factors affecting atherosclerosis:
- smoking
- high cholesterol
- heredity
- hypertension
- stress

Cocaine use is associated with infarction. From the records, your husband has not used cocaine, but opioids. Opioids are a different subgroup of drugs which aren't associated with heart attack. So, I do not find any connection between the medications he was using and infarction.

Hope it answered to your queries!

Thank you very much, yes this answers what I wanted to know. Do you mind if I ask one more question? Not sure how to ask this, however, the combination of the medications, particularly the clonazepam 1mg 2-3 times per day or more cause sudden cardiac arrest with his heart condition? To be very honest with you, the coroner DID NOT ORDER ANY TOXICOLOGY AND DID NOT DO AN AUTOPSY. The coroner also refused to talk to me or review any medical records. Do to the medications found he put "sudden cardiac arrest due to prescription drugs" on the death certificate, due to suicide. My husband was a God fearing man, he would not take his life. The police confiscated all meds and due to the prescription and amount of pills left, all were accounted for except the clonazepam which out 90 there were 70 left.

Detailed Answer:
Hi back,

Clonazepam 1mg taken 2-3 times a day never can cause sudden death. This doses are within therapeutic range and never can cause such a thing.

We don't know for sure the exact dose he had taken that day (we can suppose around 20 tablets) and an autopsy and toxicological study should have be done in his case. This would have established an exact cause of death (It was from overdose or by other medical causes). Forensic medicine is an exact discipline and doesn't tolerate supposition and uncertainties. Talk with your lawyers about law possibilities to ask for autopsy and toxicological study.

Hope this was of help!

Thanks again. Unfortunately, since the coroner did not order an autopsy, if I wanted one done, I would have had to pay for it upfront my self. I did not have the money to do that, there was no life insurance on him and he was cremated. The coroner stated that the blood had clotted is why he did not do toxicology but I know that it can also be done by hair or finger nails. I have talked to several law firms as far as medical malpractice and no one will take it because it is not a high dollar case. I have one more firm I am going to try. However, as far as the coroner goes I had made some inquiries and even sent him a certified letter with legal terms which were given to me by an attorney and got no where except our City attorney sent me an email stating that there was a detailed investigation done, which is not the case. The coroners office never even followed up with the police on their findings. I haven't been able to find a law firm to take on the coroner case. I don't have funds to hire an attorney. Thank you so much for talking to me. I just want some justice for my husband. Thanks again.
