Can HIV Be Transmitted Through Protected Intercourse?

Question: Hi doctor,

I am a 24 yr old male who recently had protected sex with a sex worker (unknown hiv status -assume she is hiv +). After cumming inside her ( was wearing 2 condoms) , I slipped my penis out and I might hav touched the outer part of the condom with my left thumb to remove it and then I went to wash my penis. I washed it using the same left thumb (there might hav been a 5 seconds gap between touching and washing). I rubbed the penis opening to wash it using the same left thumb. Exactly after three weeks , I am experiencing illness and no other symptoms. Please let me if there is any chance of me to acquire HIV from this experience.
2. I had an itching below my scrotum for few days and I use to scratch it before I went to this sex worker, any chance the vaginal fluid might have dripped out and came in comtact with this itchy part (which might hav had a small opening due to scratchinng - not sure). Please share your thoughts.

There is very little possibility of the HIV infection.

If the partner is having high viral load of HIV and exposure is on penile mucosa than and than only HIV can be transmitted.

only scrotum itching won't be HIV or other sexually transmitted diseases. It must be associated with other feature like lymh node swelling, or fever or discharge.

Thank You for answering my question at such a short notice. I do have few doubts regarding the above encounter.

1. As i had a little amount of Vaginal fluid on my thumb and i used the same thumb to wash the penis opening, Is there no way i could have touched the Penile Mucosa? Theoretically speaking, there was a contact of vaginal fluid from vagina to thumb, and then thumb to penis opening (less than 5 seconds between touching and washing as I have already mentioned). Even If I washed my penis opening thoroughly, does that mean I might not have touched the penile mucosa?

2. The scratching I mentioned is not a symptom Doctor, the scratching has been there for few weeks before my sexual encounter. I just wanted to clarify as I have been scratching the bottom of my scrotum before my sexual encounter, is there any way her vaginal fluid might have leaked out of her Vagina while intercourse and came in contact with the scratch area. The last time I scratched might have been 24 hours before the sex. Does that mean even if there was a cut or opening, it might have been covered naturally, thus preventing the transmission? or is there no chance of transmission at all once the vaginal fluid came in contact with air? I read in few forums that HIV cannot get transmitted outside the body (it dies in air) and it is not possible to "stuff HIV virus" inside the urethra without penetrative sex, is that true? And I am experiencing the fever and cough just now for which I am slightly concerned (no other symptoms till date).

3. If I need to take a HIV test (just in case to make me feel more sure). It has been exactly 3 weeks. What can be the proper time interval to take it after the encounter and what will be the test name? Please advise.

(sorry for the lengthy post and please bear if the questions I ask are silly)

Thanks for the confirmation. Please find my final questions and requesting you to clear them one last time for now.

1. After having sex and when i washed my penis tip with the left thumb, there was definitely some residual sperm at the tip, so there is no way that the vaginal fluid must have transmitted the virus from itself to my penis through the residual sperm content? or the virus would have become ineffective as soon as the vaginal fluid was exposed to air? If it is true that the virus dies immediately after exposing to air, its a relief. please advise.

2. Will the accuracy of PCR be 100 percent doctor? or what other tests should i perform and if so, after how many days should I perform the PCR/other test to make the result 100 percent correct. kindly let me know.

3. I had fever these 2 days and Now I feel alright. There was no other symptoms that you mentioned before till date except fever and cough with phlegm these 2 days. Is there any chance this might be a symptom or is this due to some other reasons. plz share your thoughts.

4. I felt comfortable sharing these experiences with you. thanks again! Is there anyway I can request the site to directly address my questions to you in the future? please let me know. And Please give me few advises that might help me in the future with similar scenarios.

1. Practically for this point there is no possibility of HIV in you, but for mental relief you can get yourself tested with HIV PCR.

2. Yes PCR is almost 100 percent accurate when tested in proper time.
