Based on your query, my opinion is as follows.
1. Fasting sugars and postprandial glucose are not in the normal range.
2.This means your diabetic status is not controlled and the dosage of medicines need to be increased.
3. Presence of glucose in urine is also not a good sign, it can lead to renal complications if not controlled.
4. Diabetes for 12 years uncontrolled can cause long term complications, so it needs to be controlled as soon as possible.
5. Also get a HbA1C done which will show diabetic control over past three months.
