Hello, and I hope I can help you today.

There are a few possibilities of what can cause back pain in a term pregnancy.

The first, and most obvious, are contractions. If your baby is lying in a certain way inside you, the head could be pressing on your lower back from the inside and you may be having false labor. This is when contractions are irregular and come and go but do not produce dilation. When women have contractions they get winded and feel the need to breathe heavily.

The other possibility of what can cause pain on one side of your lower back is a kidney stone. Passing a kidney stone is supposed to be as painful as labor, and while your kidney cramps the pain can be very severe. Kidney stones are more common on the right during pregnancy because of the position of the uterus inside you. Also people usually feel the need to squirm around or move while they have the pain because they are trying to get comfortable but there is no way to do so.

There are other possibilities of what could be causing the pain, but the safest thing to do if the pain continues is to go to the hospital for an evaluation, and let your OB provider know your situation in case they want to evaluate you personally. The hospital can check to see if you are having contractions and test your urine for any signs of kidney stone or infection. In the meantime drink lots of fluids to flush your kidneys and you can take Tylenol (Acetaminophen) or Paracetamol at the normal adult dosage for pain.

Also make sure your baby is moving and at normal intervals during this time. If the baby is not moving well, or you develop any vaginal bleeding or leakage of fluid please go to the hospital urgently for evaluation.

I hope I was able to answer your question and that you follow my advice and see a doctor if the pain continues.

Best wishes for the rest of your pregnanacy and delivery, Dr. Brown
