What Causes Heavy Menstrual Bleeding And Vaginal Itching?

A couple months ago I had a very heavy period with cloting that lasted three weeks. I had a pap test and it came vack normal. Last month and so far this month no period at all! I am 45 yrs. old. I get hot flashes, I have discharge and an itchy vagina on the inside and out. Is this normal?

Thank you for your query. I understand your concerns.

From what you have written think you are in the perimenopausal stage. Within a few months-a couple of years you may become menopausal and stop seeing your periods altogether.

Perimenopause means that a woman’s ovaries are slowly failing to produce the female hormones oestrogen and progesterone. When she attains menopause, her hormone production is almost negligible.

When a woman is in perimenopause her periods become irregular, she may not see a period for a few months and then she has a period which could be heavy with clots. She may even see 2 periods in a month. So, she has menstrual irregularities. She also has hot flashes, night sweats, mood changes, dryness of vagina etc.

Dry vagina can lead to vaginal infections as the protective barrier of the vagina is broken. As you are having a vaginal discharge and associated itching, you are likely to have a vaginal infection, likely fungal. You can use Clotrimazole pessary (vaginal tablet) 500 mg. It is a once only dose. Insert the pessary inside your vagina.

If your periods are very heavy with clots, your doctor can prescribe Norethisterone 5 mg three times/day for 7-10 days. This will reduce the bleeding and stop it altogether.

To reduce hot flashes you could take non-hormonal medication like Clonidine 100 mcgs twice a day and gradually increase the dose until you see relief from your symptoms. You could also take Venlafaxine or Gabapentin for your hot flashes. If your symptoms are too severe, you could also be started on HRT –hormonal replacement therapy.
