Trying To Conceive. Diagnosed With Polycystic Kidneys And Ovaries, Hypertension And Elevated Thyroid. Taken Metformin. Fear Risks?

I have been diagnosed with bilateral polycystic kidneys, bilateral polycystic ovaries, hypertension and elevated thyroid.....I am trying to concieve. The thyroid is under control now..... I wss prescribed siphene 100mg followed by newmon R 75 iu injections for 2 days...... this did not work so the dose was increased to 150 iu...... in all I have taken 10 shots for ovulation induction in one cycle combined with clomiphene tablets..... since nothing happened laproscopic ovarian drilling was done and emdometrium biopsy was done..... the result is non tubercolosis mycobacteria is detected. For this I have been prescribed r cinex...... so currently I am on thyroid medication + metformin + r cinex and trying to concieve.... I need to know if so much medication is harmful for my system especially the kidneys and if it is alright for me to try for a baby while on so much medication?...... also what are the risks to the baby in case...... thank you.

I am Dr Abraham and I understand that you might be under a lot of stress due to your multiple problems.

Elevated thyroid, PCOD, Non Mycobacterial infections can pose a problem to your fertility. I think 10 shots of ovulation induction with Siphene is more than enough with medical management. Polycystic kidneys may cause you hypertension due to declining renal function. So it is important to have your renal function checked frequently, particularly when you are becoming pregnant. What medications are you taking for BP? If you are trying to conceive labetelol or Methyl DOPA are the preferred ones as they dont have damaging effects to the baby. But your hypertension needs to be contrlled as increase in our BP can compromise the blood supply to the baby.

R Cinex does not harm the baby so you van continue that. Metformin is needed for PCOD, especially if you are obese and it doesnt seem to harm the baby either. You have to supplement yourself with vitamin B12 as metformin can reduce your vit b12 levels.You will have to discuss with your GP/ Endocrinologist/Nephrologist regarding BP control and optimizing other medications.

For your infertility problem I hope IVF (In Vitro Fertilisation) with ICSI technique will be the best. If you can freeze the embryos you dont have to undergo induction each time. This is a procedure where your husband's sperm is injected into your egg. You need to check the Ovarian reserve before that, by a test called AMH or Anti Mullerian Hormone which will predict the Ovarian response to induction.It is better to have an appointment with an Infertility Specialist and discuss the matters.
