Ongoing Blood Loss. History Of Heart Failure, COPD, Kidney Disease, Irregular Heart Beat. On Warfarin. Risk Of Stroke If Taken Off Warfarin?

Hi tomorrow I'm meeting with my mothers doctors she has an ongoing blood loss which they found today in the small bowl (we think ) still to be confirmed she has had 3pints of blood in 9 days she has a lot of medical things wrong eg.. Heart failure , COPD, chronic kidney disease ,under active thyroid ,suffers with low blood pressure , has an irregular heart beat hence why she on warfarin 1mg daily plus all other medication ,the talk was to take her off the warfarin for couple of days to do a procedure on her heart for the irregular heart beat to take her off the warfarin for good my concern is the high risk of a Stroke what do you think

Dear Ma'm
The risk of stroke in an average patients with atrial fibrillation {AF}(irregularly irregular heart beat) is ~ 4.5% in one year. This is reduced to ~1.5% by warfarin therapy and to 2.5% by aspirin therapy.
However the risk is determined by many parameters in a given patient. It is more for certain conditions associated with AF like mitral valve disease or prosthetic heart valve.
Whenever they do procedure on heart for AF they stop warfarin and replace it with heparins. Last dose of heparin (LMWH) is given 24 hours prior to procedure.
Since there is an ongoing bleed it would be prudent to check the INR and take a decision after considering everything.
