Hello,
Thanks for the query to H.C.M. Forum.
Congestive heart failure is already , diabetes ( insulin dependent ).
Weight gain 70 lbs 8 months ( approximately).
Edema ( swelling) over dependent part i.e. ankle & legs .
Now you stated that there is fatigue.
Fatigue is due to retention of fluid.
May be due to anemia ,
May be due to chronic renal failure as all symptoms are indicating towards that side.
In my opinion consult a physician at one and get his opinion as condition is not good.
Good luck.

General & Family Physician Dr. Pavan Kumar Gupta's  Response
Hello,
Your symptoms of chest pains on exercises, fatigue, tightness of throat on exercises along with swollen ankles and gain of lot of weight clearly suggest water retention due to congestive heart failure.Uncontrolled diabetes in long run results in various cardiovascular complications.
You must immediately consult a cardiologist and get properly investigated by getting ECHO and other tests like angiography and renal function tests.
Treatment lies in appropriate medications for cardiovascular complications and meticulous control of diabetes and serum lipids.
Thanks
