After Treatment For Typhoid What Causes Fluctuations In Blood Platelets Count?

Dear Doctor,

My name is sunita, aged 41, unmarried female. In August 2010, i had an episode of typhoid. After my cure, i had the same episode in March, 2011. After rest and medication, i resumed work. But, exactly after 1 week, i started getting fever, my cbc count showed low haemoglobin, esr was 70, and platelet count was 91,000. Ultrasound showed slight infection in liver, gall bladder and spleen, my chest x-ray is normal. Based on this test, doctor gave me multivitamins, but no antibiotics, as he said they were not needed. My next tests showed raised platelet count of 2.47, and esr as 20. Now, yesterday, i got my cbc esr done, which shows esr as 69, and platelet count as 1.50. I am worried by these fluctuations. Please advise.

Platelet counts can be reduced by a variety of causes including infectious diseases caused by bacteria and viruses,certain medicines,inflammatory conditions but the count returns to normal level unless there is any problem in bone marrow.Your counts are probably lower than previous because of some inflammatory condition as your ESR is high..If you are feeling good nothing to worry!take care!

Thanks for asking.
Based on your query, my opinion is as follows.
1. Infection of bone marrow by typhoid bacteria can lead to aplastic anemia, leading to reduced platelet count, anemia and even leukopenia.
2. However, it seems you were suffering from possible viral infection, which led to low platelet count. As it was viral in nature, no antibiotics was prescribed.
3. There is always fluctuation of platelet counts. Not to worry until it is in the normal range of 1.5-4.5 lakhs/cmm.
