65 Years, Have High BP. Ultrasound Report Shows Bilaterally Increased Grade 1 Renal Cortical Echogenicity. Explain

Hi Doctors,
I want to know is bilaterally increased grade 1 renal cortical echogenicity means ? My mother who is 65 years old and high blood pressure has been diagnosed by this. Her urea is 38 where as the normal range is 1-20, her creatinine is 1.35 whereas the normal range is .50 - .90 , her uric acid is 9.56 whereas normal range is 2.4-5.7 , her ultra sound report says she has bilaterally increased grade 1 renal cortical echogenicity. Please tell me what do all these tests show ? I am not a doctor, please kindly explain to me in a layman/ simple language.
My email is YYYY@YYYY
Thank you

Hello,
Thanks for the query to H.C.M. Forum.
I would try to explain you in your own and simple language .
Increased renal cortical echogenicity means renal tissues are mildly destroyed ( borderline case or near to chronic renal failure ,if not treated).
Blood urea is 38 ( as you mentioned that normal range is 1-20 , while in standard books the normal range of urea is 20-40 ) .
Serum CREATININE is 1.35 ( normal up to 1 , but in few books it is 1.35 ).
Uric acid level is 9.56 ( normal 2.4 -5.7 ).
Uric acid if high in level it may damage kidney and that may be the reason of renal cortical damage ( echogenicity ) .
Serum creatinine level is very very important if it elevated chances of renal failure increased.
In my opinion plenty of fluid with diuretics will help .
In such type of cases I have treated my patients with diuretics and plenty of fluid orally or intravenous .
Consult a nephrologist and get his opinion.
Gout also develops due to high level of uric acid so take treatment for this also.
Good luck for your mother. Dr. HET
