Why Can't I Lose Any Weight Despite Following A Strict Diet And Exercise Regime?

Question: Dear Doctor,
Let me begin by saying that I am 28 years old, my height is 5ft8inch, and weight is 84kg. Last year around this time I was approximately 71kg and used to go to the gym at that time last year.
Due to workload, unbalance of Diet and less physical activity and lots of stress, I put-on nearly 14kg in 1year and 2 month.
From last one month, I was on a strict Diet (mostly take boiled food, minimal crabs) and also following the strict instruction of the certified trainer. Having said that, I haven’t lost a single pound of body weight in last one month. As far as my knowledge is the concern (All hormone tests are being done and found normal Cortisol, Thyroid, and Blood sugar level, Liver panel Test, Testosterone, etc) this is being happened due to genetic curse. I would simply blame this on the Genetic Parts.
How much time do you really think to lose one kg? How much time do you think to lose 14 kg overall if a person follows strict diet and instruction of certified trainer (If the hormone is normal, medical history also being normal)?
what extra do you think one should do to lose one kg in a month as I am not setting the big goal? I was setting small goals like 2 to 3 kg in a month. Did I set the goal wrong (2 to 3kg in a month)?
Here are the very tough questions, which Doctors are not comfortable to answer due to lack of knowledge, which is the Genetic cause. How to find which Gene is malfunctioning so that we can eliminate the root cause of body not responding to weight loss training?
How to suppress that particular Gene in order to maximize the weight loss process, and enhance the muscle building. Here I am not talking about any boring Diet and Exercise which a school going kid also know. Please don’t give suggestion on diet and exercise
As I know without Genetic Help the Exercise and the diet have to have an almost negligible effect on weight loss and muscle building. I would Consider Genetic as supreme in terms of muscle building and weight loss and in my view and through extensive studies, I found the diet and exercise a very easy thing for easy people to suggest.
What suggestions and advice would you like to give in terms of Hormone/Genetic/excercise/ last but not the least food, so that I could lose weight and build muscle as I already said, my body is nor responding?

Hi,

I would like to answer your questions one by one -

How much time do you really think to lose one kg? How much time do you think to lose 14 kg overall if a person follows strict diet and instruction of a certified trainer (If the hormone is normal, medical history also being normal)?

---- One can lose upto 1 kg per 2 weeks in a healthy way, if the diet is right. Right diet means less number of calorie intake compared to calorie expenditure.

A person with sedentary life style would need not more than 1500 cal per day. Considering that fact if one consumes 1500 cal per day and workout in gym spending around 500 cal per day in excess can lose weight upto 500gms per week. It is always the calories-in and calories-out we have to concentrate on.

What extra do you think one should do to lose one kg in a month as I am not setting the big goal? I was setting small goals like 2 to 3 kg in a month. Did I set the goal wrong (2 to 3 kg in a month)?

--- A goal of 1 to 2 kg per month is fine. It is all about diet and exercise, you can fasten the weight loss process by taking caffeine rich fat burners available in supplement store. But most of them can cause side effects, specially ones which have Synepherine and other catecholamines.

-- The fact about the genetic aspects in weight loss or gain, most of the theories and studies point at the genes involvement in weight gain, but not prevent weight loss when one is on strict diet and exercise.

I am very much Impressed, the way you answered my questions step-by-step.
However, there are some suspicions still existed, I would like to get it clarified those by you.

1)I very politely differ with you on some points, what do you mean by One can loose up to 1 kg per 2 weeks in a healthy way, if the diet is right, That means only Diet is solely responsible for weight loss(Excluding Hormonal Profile, Genetic Influence, and Excercise, Stress level, Environmental factors)?

2)I don't think Diet is only responsible for weight loss, it's the physical activities of that person, the medical condition like the hormonal profile, stress level, sleeping pattern.
I generally debunk(not completely but almost) the theory of calories deficit it has to have a very negligible effect. If a genetically fat person finding very very hard to lose weight if his/her family history appeared to be obese.In that case, your so-called Right Diet has to have the very negligible effect. Here I am not talking about it is impossible for that person to lose weight.

Genes can affect fat storage and fat loss by influencing energy intake, energy expenditure, or nutrient partitioning. Researchers have coined the term "obesogenic environment" gene also affect the metabolism(Weight Loss)

Similarly, A genetically thin person finding it extremely difficult to gain muscle mass as the satellite cell surroundings the muscle fiber cells are not being fully activated which is again the genetic cause. So he will find extremely difficult to build muscle-mass but not impossible but in the extreme process he will get frustrated and quit.

--- You are absolutely right, but one should think to what degree. Does it make the diet effect negligible, In my opinion NO. Diet is important, genes may have effect , negligible in many and considerable in some. Even if considerable, the diet still is an aiding factor in them.

--- Stamina is gained only after some time, it may vary from one to another. Keep up, and increase the insensitive of workout slowly. Fat burners will surely show a significant change.

--- I assume your cortisol levels are normal. There is no option of spot reduction in weight loss plans. The more you loose the more they shrink.

--- Weight training will reduce fat from all pats of the body. The above are the last ones to loose. First you loose fat over active parts of the body, staring from face arms and so on.
