Question: Hello Dr. Could you please tell me what you believe this scaly skin is on the head of my penis? It does not itch but it is very painful. After your initial response I will go into further detail. I will send over pictures.

Hello Dr. I am 38 years old. About 2 months ago I developed a patch of rough skin on the bottom of my penis head. At this time it did not hurt. A few weeks later it got bigger and began to hurt. These spots never itched, just hurt. Now they are scaly and my whole penis head hurts. At the same time as this I broke out in rashes on my forearms and leg. These rashes itched and disappeared. I also had burning urination and now it burns after I urinate. All of my STD tests came back negative...I got tested for them all during this time. My dermatologist thinks it is penile eczema, but I'm not so sure since this spot never itched. She prescribed me Elidel cream which hasn't really helped much. I also leak quite a bit of urine after urination which I've always had a problem with. I see my urologist in 2 weeks. They are thinking I may have prostatitis...but that wouldn't explain the scaly skin or rashes. I haven't seen anything like this on the Internet. It is ruining my life. Do you have any thoughts to what it could be? It's not from a condom or sex or laundry detergent. I am at a loss. What are your thoughts about my dermatologists prognosis? Does I look like any form of std? This has never happened to me before. Thank you. I have been using the elidel cream and eucerin skin calming lotion. Before that I was using neosporin and some hydrocortisone cream. Nothing works or brings relief. Your thoughts are appreciated

I believe it is a fungal infection.
Usually fungal infections are itchy but I have seen few cases which presented like your.
I advice you to use an antifungal cream like clotrimazole and see the response.

We suspected fungal infection as well, I took a prescription of fluconazole with no improvement so it is not fungal. What do you think of my dermatologist's assessment of penile eczema? Wouldn't that itch? And why have I broke out in rashes during this period? Does it look like an std? If so which one? Do you think it's related to my prostate issues and leaking of urine? If so what would be best to put on it? Please leave a detailed response...thank you!

First of all, it is not a std.
None of the std presents like this.
Eczema as you correctly mentioned is itchy and oozes too.
The most common diagnosis would have been fungal, which is ruled out now.
Next common possibility is psoriasis as you have body rashes too.
I would like too see a photograph of body rashes if they are present now.
