Question: Pus cells 18-20 WBC HPF. LEUCOCYTES ESTERASE POSITIVE ARE A FEW FINDINGS OF MY DAUGHTER'S URINE EXAMINATIONS DURING 7th MONTH OF HER PREGNANCY. ESR 63 MM/ HR. VITAMIN D 25 HYDROXY SERUM CLIA- 22.70. KINDLY GIVE YOUR OPINION.

I am Dr. Jyothi BL and I will be answering your query.

Based on your query, my opinion is as follows:

1. It appears to be urinary tract infection. (Pus cells and leucocytes esterase positive)

2. UTI are more common in 3rd trimester due to compression of ureter and also dilatation of ureter, which slowly urine flow and increases risk of infection.

3. Antibiotic selection should be based on urine culture sensitivities. She can be started on amoxicillin or cephalexin, etc. You can take prescription from your doctor.

4. ESR level is elevated. It is commonly increased in second and third trimester of pregnancy. Along with possible anemia and present infection, it is raised. Not to worry. It will come down, once infection is under control.

5. Vit D is mildly reduced (25-70 normal values). She will require Vit D supplements along with good sun exposure. This should help her recover.

Dear doctor

Thanks for having responded.

I would like to inform that my daughter had a seizure when she was a app. 12 years. The seizures would not recur unless she took stress due to any reason. And, there used to be long gaps between the two seizures. Now, she is on her family way in the 7th month of her pregnancy. At one point of time, she was advised by the doctors to stop medicines for the seizures before she married when she was 25 years, after all her reports appeared normal.

But, unfortunately, she had seizures twice after her marriage- the last seizure was app. one year back. Now, she is taking TAB. TEGRITAL 200 mg. twice in a day.

Has this problem any thing to do with these medical reports, or, if these medicines affect the body, especially during pregnancy? In fact, she is taking this medicines in the above dosages after it was advised by the neurologist, who knew the history of the patient from the beginning. The neurologist was consulted even before she thought of conceiving.

No, Tegrital is not responsible for any of the present symptoms.

Usually anti-epileptics are stopped by slowly reducing dose after 5 years of no seizures.

As it was a recurrence, she was started on Tegrital. It is mostly safe during pregnancy.
