Question: Hi Doctor, what is the mean of "increased parenchymal echoes with poor CMD" in ultrasound report

The meaning of increased parenchymal echoes is that the kidney is more whiter on ultrasound scan than normal.

Poor CMD means that the differentiation between the layers of kidney is not clearly visualised.

The above findings show that there is a kidney condition which requires blood tests and detailed evaluation.

Ceratinine 16 is high and recovery depends on the diagnosis made clinically.

please find attached herewith reports even include today's as well. for the next step local doctor said they will do the biopsy of kidney tissue for that they tried today to take the sample but they could not they said because of fat, I don't understand exactly what is going on because they are not giving any clear statement. almost every day they are doing dialysis.

looking for your advise and if you could tell me what exactly she is suffering with.

Right now it is important to do kidney biopsy to establish diagnosis

Thanks for sharing the reports.

From the reports it is clear that the serum creatinine and blood urea levels are higher and the eGFR which is filtration rate of kidney has decreased.

In simple terms the kidney is unable to filter out the toxic waste and which is remaining in blood necessitating dialysis which is a procedure to artificially filter the blood in a machine.

In such patients it is important to establish the cause of kidney problems. The cause can be kidney is affected due to long term complication of diabetes or due to the effect of any recent infection since ASLO level is increased.

To know the possible cause, you have to get kidney biopsy done. It is a simple procedure and involves taking a sample of kidney tissue using a slightly bigger needle when seeing on ultrasound scan. I myself have done kidney biopsy in many patients with the kidney specialist doctor and in most patients it was possible to get a sample easily with less pain and discomfort to the patient.

In my opinion, the kidney biopsy becomes important to confirm diagnosis and before starting treatment. If there is some difficulty in doing biopsy in that hospital then she can get the biopsy done in any place where they do it regularly and without giving much pain to the patient.

The biopsy will tell the disease condition and whether it can be treated or if regular dialysis or kidney transplant is a long term solution. In many patients there is good response to certain kidney conditions after confirming diagnosis. Therefore please tell your family not to lose hope and continue with kidney biopsy and treatment after that preferably in a kidney specialty centre in Lahore or any place close to her residence because kidney patients require frequent visits to hospital for regular health monitoring.
