What Does This X-Ray Report Indicate?

Question: My mother, 60 yrs has recently been detected to have "osteoporosis" and in the X-Ray report it is written that there is "degenerative changes in the lumber spine".
For osteoporosis, she is taking "Terifrac" injection.
What about that "degenerative changes in the lumber spine"?-is it the same as "degenerative disk desease"?
She feels pain in the lumber region and the left leg almost all the time.
What is it and what ot do.
please suggest.

Hello,
I have studied her reports and I want to tell you that apart from terifrac she will also need calcium and vitamin D 3 regularly. She should also take calcium rich diet.
Her degenerative changes in spine is common with disc disease.It is age related changes in spine and it can not be cured completely. Her treatment of osteoporosis will be helpful in her back pain also. Apart from them she should avoid forward bending, ground level sitting and squatting.
She will also get benefit by lumbosacral belt and hot water fomentation and volitran gel.

She has got Vitamin D3 value=91(normal range is <65).
and calcium value=10.1(normal range<10)

Secondly you might be knowing that she had ORIF operation of olecranon with TBW and 2-K wires 7 months ago.
movement of the hand was fine but pain and stiffness is still there.
But i notice that, after starting of TERIFRAC, the pain in the hand is increased(i doubt: it may be a coincidence).
I would like to take the metals out within next 3-4 months.
Will the TERIFRAC injection create any problem(i mean whether it will be jammed!).

She is having pain almost everytime, even when she is sleeping!
what the problem is?

RSD is causing her pain

Hello,
If you remember we diagnosed her as RSD. In this disease there is pain in the elbow most of the time and this is causing her trouble at present. It is not related to terifrac, you can safely continue that injection.I would suggest you to start tablet Gabapin NT.

OK, what about the pains in the lumber spine region? why pain is there when she is in sleep?
And what about the pain in the legs? is it for RSD or it is coming from the "degenerative changes in the lumber spine"?
If from lumber spine problem then what is the treatment?

Hello,
If there is back pain with leg pain then she can get MRI of the spine to confirm the diagnosis. If there is nerve compression then it can cause such pain and it is not related to RSD. But osteoporosis can cause pain in the spine.
So I would recommend you to start physiotherapy of the back and start tablet Pregabalin M at night time.

Sir,
I have visited the local orthopedic doctor here.
HE has given "Nortipan" medicine one everyday at night time. Now my mother feels slightly less pain. But every morning her legs,hands and lumber region get stuffy.
Moreover, if she is on her feet for some time, pain in the lumber region starts.
the doctor has not given MRI test.
Secondly, she does not have any appetite-almost all the time she does not like foods.Gastroenterologist here checked and found nothing.
What is your opinion on the loss of appetite and the nature of pain.

Hello,
This tablet she is taking is having tryptomer and it is for sedation and nerve pain. She should take it for some time may be at least 1 to 2 months. If her pain continues then she needs MRI.
As far as her loss of appetite is concern , it can be due to generalised weakness or taking medicines which some time can alter taste of tongue. I would recommend you to take LIv 52 tablet one daily which can increase her appetite.

She is taking the following medicines daily:
cetapin XR for blood sugar
Amtas 5 for blood pressure
evion LC
Terifrac injection
uprise D3(1k)
ultracet
nortipan(75 mg)

1) which medicine is creating loss of appetite?
2) even if her vitamin-D3 level is 90(where normal range is <60) why "Uprise D3(1k)" is given?
3) whether "nortipan" is going to cure anything?

Hello,
If her vitamin D is normal then stop it. Because excess of vitamin D get deposit in liver and it can cause loss of appetite.So I would suggest you to avoid uprise D3 and terifrac.
Nortipan can improve her pain specially in her age when normal pain killers are toxic. Ultracet is also cause of loss of appetite.

Thank you for the suggestion.
I would like to mention two things:
1) Her D3 level was low-it is after two months of "uprise D3(60k)-weekly", the level is increased.
2) After she is detected "osteoporosis", the "Terifrac(20mcg)" was stsrted.

Now i would again like to ask for your suggestion on:
1) Whether "Terifrac" should be stopped-if yes then what alternative should be taken?
2) Should "Uprise D3(1k)-daily" should be stopped?

As her vitamin D level is normal so there is no need to give her uprise D3. Terifrac is used for osteoporosis but if vitamin D is normal then there is no need to give this medicine, as it can cause side effects also. There is no need for substitution.

Hello,
Osteoporosis will improve as her vitamin D level is normal now. We can not give her more vitamin D as it will cause toxicity then.
Teriparatide should not be given if vitamin D level are high as it can also cause side effects.
As far as her degenerative spine disease is concerned she will feel better with lumbosacral belt and physiotherapy. Vitamin D or Teriparatide is not direct treatment for it.

Parallely the doctor here has given "Evion LC" also. I would like to listen whether i should continue that or not.

If after some days it is seen that her "vitamin D3" level has been low, whether that can be simply made normal again by "vitamin D3 medicine" then?
