What Does The Following Mammogram Report Indicate?

Question: Hello doctor,

This is XXXX, my mother who is 56 years old felt a lump like in her right breast two days ago for which she consulted a local gynaecologist who suggested for a mammogram and the results came like "Focal edematous mammary lobule with a hypoehoic area measuring 6.7*4.3 mm with minimal peripheral vascularity noted in the UOQ at 9:00 clock position of the right breast . Rest of the breast appears normal. Few prominent axillary lymph nodes with preserved fatty helium noted on both axillas , largest measuring 14.1*6.9 mm on right side and 11.6*7.3 mm on left side . Focal fibroadenisis(?) , mass(?). What needs to be done now . I am very scared for her . What is your expert opinion on this I have attached the reports further . She is a diabetic and no patient and for the past three days her sugar levels are elevated . It never elevates for her even when she checked 15 days ago it was normal well within range . Is biopsy required . When I touched her lump physically it was neither too hard nor too soft but it was kind of moving but not rolling but still it was not fixed like ( I couldn't hold it to together that is what I meant earlier) . Her paternal aunt died due to stomach cancer before she was born other than that no history as such . Also it's been six months since she got her last periods I think she is in her menopausal stage . Please help us . I would be really helpful if an oncologist answers this question

Hello dear. I have gone through the details and seen the reports. So it depicts a breast lump which is not very well defined and hence a biopsy is justified to diagnose it accurately. The nodes by description appears normal. So as the age increases there is a risk of breast cancer in females irrespective of family history. She must undergo a biopsy.

Hello doctor,
Thanks for your reply , but based on your experience do you think it has chances of being malignant? Also she just did her physical checkup on January where the gynaecologist said the breasts are normal with no lumps , also only for the past three to four days her sugar level is elevated can we consider any infection also because they generally say that insulin patients get their sugar level high if there is an infection in the body . I am scared

Hello again.

So we have seen cases in which the lump was barely palpable but ultrasonography detected it. There is no risk in biopsy and this is the only sure shot method to rule out malignancy. It is uncommon to have an infection in breast even if the patient is diabetic as there should be other signs of infection too like fever.

It is also quite possible that the lump was not there in January and now it's appeared. Please go for the biopsy.

Hello doctor,
Happened to meet a general surgeon today who checked my mother and examined her he said the lump is neither attached down nor to the breast tissue more feels like floating also the size is very small but still he said that we should go ahead with the laparoscopic surgery and we can give it for biopsy , the blood tests and the other echo and ecg came back normal

Yes, a biopsy is a must

Hello dear.

A biopsy is the first step and there is no role of the surgeon at present. A biopsy can easily be done by ultrasound guidance. Please remember, if malignancy is proven then only surgery should be done and that too only by a cancer surgeon (Onco-surgeon) and not by a general surgeon as principles of cancer surgery are different and to avoid any suffering on the patient front.

Moreover, I don't understand what a laparoscopy will do here. Laparoscopy has no role in breast cancer.
