What Does The Following HIV Combi Test Report Indicate?

Question: Doctor,

I am 38 years old male from dubai. I had an unprotected sex on september 16th 2014. From the next day, i feel headache and it last for 2 month. On October 21st i have done HIV Combi (I&I Ab +p.24 Ag ) test and it's result was .14. From starting onwards, i feel little temperature in my body (it continuing now also). But when i tested, i dont have fever. Now i feel constipation for last 2-3 months. My motion is happening on 1-2 times in a week and i also have gastric problems. Last month i feel, heavy neck pain and it lasted for 2 weeks. Now also i have little neck pain. Last January 28th, i have done the HIV Combi (I&I Ab +p.24 Ag ) test again and the result was .19. I have checked lymphocytes. The results,
Neutrophils 42.5 % 34.0 -67.9

Lymphocytes 41.8 % 21.8 -53.1

Moncytes 8.6 % 5.3 -12.

Eosnophils 6. % 0.8 -7.0

Basophils 0.5 % 0. -1.2

Now i saw some fungal infection in the head of penis and it itching. After washing there, i will not feel itching for some time. When fungal come, itching will start. There is no discharge from penis.

Is there is any chance of HIV infection.

As you have undergone HIV 1 and 2 and P24 COMBO test at around 5 weeks of possible exposure and the test has come as Non Reactive with index of 0.14.

It means that antibodies to HIV 1 / 2 have not been detected in the sample. This means that you are either not been exposed to HIV 1/2 infection or the sample has been tested during window period (before the development of detectable levels of antibodies).

As you have repeated the test after 16 weeks of possible exposure that is after 90 days of exposure and the test as come as negative with index 0.19.

Your concern is regarding increase in index value, don't worry as your result has come negative after 90 days of possible exposure. It is 100% conclusive that your are negative for HIV 1/2 infection.

These index values can really fluctuate. If you take test several times a day and your index value will fluctuate.

So I suggest you that there is no HIV infection as you are negative for HIV 1/2 after 90 days of possible exposure. Nothing to worry.

The symptoms may be due to some other infections but your deferential leukocyte counts are in normal limits suggesting no infections.

The anxiety associated with your exposure may be causing all these symptoms.

For your constipation you need to modify your food habits by eating food rich in fibers which will help to control the symptoms.

As you are suspecting to have genital fungal infection it should be confirmed by physical examination, I would suggest you to consult your doctor for complete evaluation to confirm fungal infection and to take appropriate treatment.

I would suggest you to be careful with practices of unprotected sex, personal protection measures are very important with disease like HIV/AIDS.

Thank you for your advice.

I would like to know the actual window period is how many days. My wife and son is joining me on April 10th. Before that, should i need to do anything?.

I would like to know one more thing that, the fungal infection due to sex will come after 3-4 months of exposure.

I forgot to mention in the previous mail. Now i am feeling pain in shoulders and leg muscles always. Now the climate here is cool.

Incubation period of HIV virus is maximum of 90 days, don't worry as your result has come negative after 90 days of possible exposure. It is 100% conclusive that your are negative for HIV infection.

Fungus are the normal comensals of the genital area which will be kept under control by the helpful organisms, if there is any change in the genital environment the fungus will multiply and produce the symptoms. Due to sex also one can get the fungal infection in few days after the exposure but in your case it is too long to get this infection by sexual route.

Pain in shoulders and leg muscles could be due to some viral infection and does not suggest of HIV infection.
