What Does My Urine Test Result Suggest?

Question: I have been tested with a 24 hr urine test,
Test gas return as such: beta globulin ' gamma globulin u,gamma globulin urine albumin u,alpha 1 alpha 2-ABNORMAL
PROTEIN 192-high
KAPPA QAUANT FREE LIGHT CHAINS 22.2 high
Free Lambda light chains, Ur 1.84 high
Kappa/Lambda free light chains ratio. 12.07 high

Suspicious band in gamma region with lambda specificity.

I am assuming we're looking for myeloma at this point. What is your thought on this test?8

Thanks for the query

From the report you have furnished it does look like plasma cell disorder ( multiple myeloma). But it only says suspicious band so here is what you should do to confirm it

1. Bone marrow aspiration and biopsy
2. X ray of the skull lateral view to look for lytic lesions
3. Urine for bence XXXXXXX proteins
4. If all reports are negative and clinically there is still a suspicion of multiple myelome then I would have done a bone marrow biopsy

Hello Dr

I did not mention that I have a Stage 3 kidney disease, have had anemia as well for 3 years now, anemia is being treated with Iron i fusions as well as "as needed" epo.
Hemoglobin is still about a 9.0 with treatments and kidney disease is remaining stable with controlled high blood pressure, these factors will most likely play into the picture with possibilty of having M Myeloma i'm assuming, although what, if any difference it would make I am not sure. Your thoughts Dr?

Hello

Thanks for getting back with more info. Epo plus iron is the best way to treat anemia in CKD and it prevents blood transfusions.
There are chances that MM can worsen the CKD in many ways, most importantly calcium induced dehydration however I am really not sure you have MM at all. Please follow the diagnostics steps I have jotted down for you, be positive.
