Dear Dr. Anubha, Hi! you are a doctor, can i know your specialization.
Welcome to HCM. What looks to me is a typical case of PCOS, polycystic ovarian syndrome. you have acne and USG to support the diagnosis. How are your periods? are they regular? Are you having more of facial hair growth i..e. hirsutism. What is your BMI? >25kg/m2
If yes, then you have the typical cluster of PCOS. now to treat them what I commonly recommend my patients
1. Metformin
2. weight loss
3. a battery of tests - LH, FSH, Estradiol, 17(OH)P, Prolactin, TSH, FT4
all done in a fasting state, 8 am .

Namaskar Dr., This is XXXXXX F/O XXXX , Thank you for your early response. the following contains the details ask by you.

This is Dr. XXXX , 26 yr,Height 4.11,wt.55 kg, BMI 24kg/m2, some/very few, thin, & small hairs appear on chin.

Problem Suffering
Having cluster of pus filled bluish red pimples on chin and jaw line bilaterally,
lateral to bridge of the nose right side and 2-3 small on forehead, since more then a month.
Recently got bowls with pus discharge on both ears.

History:-
2 yr. before she suffered with acne only for the first time. After some home treatments she feel better
Her periods are regular and normal.
Three months back we have started her Q switch laser treatment to get rid for her face marks of acne and she had two sittings with kaaya skin clinic.

Treatment before ultrasound
Regular with yoga and walk and home food .(museli ,milk oats ,fruits curd etc.no consumption of oily or fried food)

Now a days she is taking medicine
1:Azithral-500od from last 21days
2. Applying KLARITHROMYCIN GEL and EPIDUO cream.

USG, Pelvic Ultrasound and its report attached (20/6/15)
“B/L Overies mild enlarged and show multiple peripheral follicles
Rt.Ov.Vol.=19.1ml, Lt.ov.vol=11.2ml”
( Rest report is Negative / Normal )

Note : from last one year I am Regular with yoga and walk and home food .(museli ,milk oats ,fruits curd etc.no consumption of oily or fried food)

Additional Q.
1.Kindly also advise when to perform/ get test done, mentioned by you ?,(means 3rd day of my periods or even before) if 3rd day, till then what is the treatment
2. shall I stop taking the medicine and applying the gel and creams
3. shall I mind sugar intake , even now I take very less
4. any food restriction & Yoga
5. will final treatment start after her test or same will continue

Hi sir, I must complement that your daughter is following a good life style.
1. she should preferably be taking her tests on 2nd or 3rd day of periods.
2. till then she can continue doing what she is doing now. I hope she has not taken any hormonal medicine in past 6-8 weeks.
3. sugars, fried food and white flour has to be avoided by all.
4. yoga and exercise is good.
5. treatment will depend upon the diagnosis and reports.

thank you valuable advise. doctor she has not taken any hormonal medicines ever. her periods are regular and normal and expected after 15 days .

She is presently worried about the acne on chin and jaw line area...its been 2 months now and they are still bluish red, pus filled and increasing and spreading all along. its very unesthetic and moreover they are very tender which is making even chewing and smiling painful. kindly have a look at the picture of acne alreday sent to you .

(again i explain that she has been applying CLARITHROMYCIN gel in morning and EPIDUO gel at night since a month but it doesnt seem to contain the acne. ) i would also like to tell you that she is a practicing dentist , so can these acne have an infective origin and also she wears mouthmask for almost 6 hours continuosly for 6 days a week...can it also exacerbate this conditon.
her height is 4'11" and weight is 57kg.

please guide
1.As explained above, please please tell me what should she do for the acne. she has already finished with the 21 day course of AZITRAL 500mg yesterday.
2.Some acne have healed but left ugly marks. what should she do for that?
3.How she can reduce her weight.
4. Shall we also take an opinion form gyenecologist or else ? i reside in XXXXXXX Vihar XXXXXXX XXXXXXX if u may refer some doctor around or XXXXXXX

Dear Sir, hi!
I understand you are worried and concerned with the present problem.
What I have recommended is a set of hormonal profile that might give us an insight into the cause of the problem.
But for immediate problem, you must see a dermaltologist or infectious disease specialist. she might need the pus culture and sensitivity from the base of acne if she is not responding to the routine antibiotics.
Kindly see a doctor who can actually look at her acne and get a culture & sensitivity done. Gynecologist does not have much role as of now.
