What Does My Thyroid Test Result Indicate?

Question: Hi I'm 15 weeks pregnant with twins. My thyroid tsh was normal prior to pregnancy. I had Hyperemisis. My tsh is now 0.006 t3 is 16 and t4 is 15.3 free 2.4. What does all this mean?

Hi XXXX,

Welcome to HCM. I have gone through the question and understand your concerns.

No need to worry. Thyroid function will be altered in pregnancy, especially if you have hyperemesis. This is due to the hormones in pregnancy like HCG and estrogen. These hormones are more in twin pregnancy. So change in thyroid function is also more in twin pregnancy.

So your TSH can be low and T4 and T3 can be high. A T4 value above 18 ug/dl is what we take for treatment of over active thyroid. Your value is 15 which is normal. So don't worry, no treatment is needed. Consider it as a normal change in pregnancy.

T3 will be high in pregnancy

Hi XXXX,

T 3 will also be high in pregnancy. Since T3 and free T4 measurements are not reliable in pregnancy, the recommendation is to do total T4 and TSH during pregnancy. This is normal for your pregnancy. We can repeat the test after 1 month.
