What Does My Thyroid Test Report Indicate?

Question: I have a surrogate finishing up her medical testing before we start meds for transfer. Our fertility doctor was worried about her thyroid. Her tsh is 1.583 and her T4 is .77 just below the range of normal from labcorps range of .82. Are those numbers a concern, and should we put her on meds for that?

I appreciate your concerns.
In clinical terms the surrogate has isolated hypothyroxinemia ( Normal TSH and low FT4). Since her FT4 is marginally low I suggest that you repeat the test again.

In the available clinical research studies, there is no consistent evidence that treating isolated hypothyroxinemia is beneficial. But we do know that low levels of T4 in overt hypothyroidism (with high TSH levels) is associated with increased maternal and fetal morbidity. Therefore in my opinion if the repeat test also shows low levels of FT4, she should be treated with low dose of thyroxine hormone.

We have done the test again and should have the results in the next 48 hours. If numbers are the same is this something that has to be taken care of before our transfer of embryos on aug 22nd or can we take care for it while we are doing the fertility drungs which we start around 30 days prior to transfer?

If the numbers are the same I suggest you start the treatment from now itself. A low dose of thyroxine is not going to harm her in any way.
