What Does My Pelvic Ultrasound Scan Report Indicate?

Question: Looking for an Ob/Gyn to answer questions on ultrasound. Pelvic ultrasound showed right ovarian volume 8.22cc (right adnexa 4.0x2.8x1.4) and left 30.9cc (left adnexa 7.2x3.3x2.5) On the report it said "Both are normal in size and appearance" .Impression from ultrasound said"Thickened endometrial echos, mid cycle endometrium versus endometrial hyperplasia or abnormal endometrial proliferation. Nonspecific left ovarian enlargement without other demonstrated abnormality." what does this mean, should i be worried about the size of the left side? Been having periods/spotting that are light but that are around 15 days long (started oct. 2014) usually have about 13 days of light spotting with 1-2 days heavy bleeding. have history of prior polyps that have been removed and history of endometriosis. 32years old 5'1 133 lbs. LMP started light on 2/3/15 BUT the heavy bleeding didnt actually start till 2/13/15 and lasted 1-2 days. I am thinking the start date of 2/3/15 is just spotting but it lasted until the start of the heavy bleeding date of 2/13/15. no other medical problems.

From your history and written report report, I feel that you have an endometrial hyperplasia.. You need a pippelle sampling to know the problem with the endometrium.. If it is fine, then hormonal pills will be sufficient to solve your bleeding problem..

Yes, your left ovary is bulky which could be a normal variant.. Just follow up after 3 months to see the size of the ovary.. If the size furthur increases you may require a laproscopy to visualise your ovaries to rule out any pathology.. If the size of ovary remains same, nothing needs to be done..

I have sent the ultrasound report, that is all i have,i can not get the images. Since i started the heavy bleeding on 2/13/15 i will say this test was done cycle day 7. my husband and i are trying to have a baby and we have had no luck yet so i dont want to go on hormonal pills. is there other ways to fix it ? could this be the reason why i have not been able to have a baby? and could this stop me from having a baby ? if i did get pregnant could this cause problems during a pregnancy?

Thanks for writing..

I have reviewed your scan report. yes, you have more trhickness than normal for day 7.. So I would suggest to get a D and c and sent for biopsy to see the cause of increased thickness..

1)You need not take hormonal pills..Instead you can take ovulation induction drug in the following cycle after consulting your doctor for 5 days from 2nd day of periods and follow up with follicular study to know the date of ovulation..Ovulation induction drugs are non hormonal pills.. Meanwhile husband's semen analysis need to be done.. Intercourse in and around the time of ovulation helps in attaining pregnancy.. This can be tried for 3 months before going for Intrauterine insemination..

Small polyps in the uterus will hinder the implantation of the fertilsed ovum.. These polyps may be misdiagnosed as endometrial hyperplasia in scan.. A D and c will solve your problem..
