What Does My Lab Test Report Indicate?

Question: Hello Doctor,

My FSH is 14.4 miu/ml (1.4 - 18.1 range) and LH is 8 miu/ml (2-9 range) and total testosterone is 278

Prolactin 11.8 ng/ml (2.1 - 17.7 range)

Estradiol 21 pg/ml (0-42 range)

T4 is 1.2 ng//dl ( .8 to 1.8 range)

Free testosterone % - 2%

SHBG is 25 (11-80 mol/L range)

i have normal beard growth. Please advise me about my report.

Hi XXXX, I have gone through your question and undestand your concerns. Your hormonal profile is completely normal.
The MRI of the pituitary gland is usually offered when there is increased prolactin hormonal levels. It is usually done to rule out pituitary adenomas which cause increased prolactin levels in the body. Your prolactin levels are completely normal, and no MRI is required based on these reports.
I would like to know the reason for your hormonal studies and your current complains, so that I can analyse better the reason for advising MRI and help you in complete way.

Thank you for the response doctor. The reason for the visit basically tiredness. Have done thyroid and vitamin D and it was appear vitamin D was low and got medicine for the same and now under normal range. Still felt the fatigue sometimes hence doctor checked my testosterone and it appears to on low end 278. Hence trying to identify the cause of it to bring it up.

Hi XXXX, thanks for responding. If your beard growth and secondary sexual characterstics are normal, then there is no reason for low testosterone. Low testosterone leads to excessive fatigue, and decreased sex drive and is usually present in older age.
Moreover, testosterone levels are not directly related to pituitary gland, and low or high levels do not warrant MRI of pituitary.
I would suggest you to get a repeat testosterone levels and if still low, do consult an endocrinologist to get properly examined, investigated and treated.Also you should get your semen examination done to assess the function of testosterone.

the testosterone was repeated twice and both the time it was at low end and hence why i have consulted endocrinologist and he ordered all these hormone test.

Based of him, if the testosterone level is low based on the negative feedback . Pituitary should generate more FSH & LH and the ranges should be very high, as its within normal ranges the doctor suggested to do MRI whether pituitary functions properly or not.

However my question is, whether the FSH os LH is under normal level or high?

Hi XXXX, it's good to know that your endocrinologist has explained you the physiology of testosterone secretion well.
Your FSH and LH levels are perfectly within normal limits, and hence low testosterone levels cannot be due to pituitary pathology.
Various studies worldwide have shown that in healthy males, the testosterone can be present at the lower level of normal range and this is normal.
Deficiency of vitamin D levels lead to fatigue, tiredness and muscle ache. Proper supplementation with vitamin D supplements improve the symptoms within few months.

thank you again doctor for the answer. I have one last question mostly, when the body don't generate enough testosterone , the pit glands should generate more fsh & lh assuming the T level is low. Does this ring any bell?

I would like to find the cause and as i wanted to increase my testosterone level to normal. I have just got the sperm count result (attached), which appears to well normal.

is there any thoughts that you can through with normal sperm count with low-t?

Hi XXXXXXX your semen analysis report is completely normal, as you have interpreted. Normal semen analysis and normal secondary sexual characterstics indicate that your testosterone level is adequate for the normal functioning, fertility and potency. Various studies have shown that some males having testosterone levels at lower range and normal semen analysis are completely normal, and in them the testosterone receptors are highly receptive, hence low levels of testosterone function adequately, and no supplements are required.
However, testosterone levels can be increased by anabolic steroids, you can consult your endocrinologist to start these drugs after fully understanding its effects and side effects.

sure Doctor, The endocrinologist suggest to use the 2 pump of 1.25 ml (1%) testosterone gel.

Having a question whether that would effect the sperm count? how long this testosterone stay if i use for one month? would the sperm count will back once i stop the testosterone gel?

Hi XXXXXXX the testosterone gel do decrease the sperm count in some men as a side effect. It usually stays in the body for about2 weeks after its discontinuation. It can take several months to regain normal sperm function after the discontinuation of the testosterone .
In my opinion, testosterone supplementation is not advised in your case as testosterone is not very low in your case and other cause for lethargy and weakness is found in your case which is vitamin D deficiency.
Moreover, testosterone supplementation has a lot of side effects.
