Question: Looking for advice on a recent chest x ray I had in the Urgent Care Center.

I can understand your situation and problem.
Your chest x ray report is showing some doubtful opacities.
No need to worry for pneumonia or tuberculosis or pleural effusion.

Your report mentioned about 7mm and 9 mm nodular lesion. This is mostly due blood vessels.
But early lung cancer can gives similar lesions.
So better to get done CT thorax with contrast to know the exact details of these nodules.
The following details about nodules are important in ruling out benign and malignant nodules.
1. Exact size
2. Margins
3. Calcified or not
4. Association with surrounding lung
5. Mediastinal lymphnodes involved or not.
And for all these details, CT thorax with contrast is needed.
So better to get done CT thorax with contrast.
If you are smoker then you should immediately quit smoking and get done chest CT to rule out early malignant nodules.

Do you think it is a good idea to compare prior chest x Ray films? I have been on surveillance for testicular cancer for the past 7 years I had many ct scams snx chest x rays. Started to treat with MRI for the past 4 years. I really do not want to undergo another CT scan for fear of additional radiation exposure. I was discharged from surveillance this past December 2014 and was given a clean XXXXXXX of health cancer fee from testicular thd orechetomy cured the problem.
The only reason for this past chest x Ray was routine because I had an upper respiratory infection. This has since cleared I feel fine. I am able to workout everyday. I never loose my breath walking or running. Thought I was and am in the best shape of my life. I am confused can you please give me some suggestions. Thank you for all of your time.

I can understand your situation and problem with additional radiation exposure.
To avoid this you should definitely compare old chest x ray with new chest x ray.
If tge opacities are similar then no need to worry.
But if new opacities are there then CT thorax with contrast is the only option.
So first compare these and then decide.
Are you having confirmed testicular cancer?
Are you smoker?

I was diagnosed with non seminoa. treated with an orenectomy right testicle was removed. Started 7 years of surveillance which the first two years included CT Scans of the pelvic region along with Chest X rays and blood tests with tumor markers. All was negative during these surveillance years and I was discharged in December 2014. My forst onset with Testicular cancer was February of 2007.
I have never smoked and I am an occassional drinker 2-4 glasses of wine per week. I am physically active the only other health issues that I have is high blood pressure and cholesterol which I am being treated for I take avapro 150 blood pressure and pravistatin 10 mg for cholestrerol and a baby aspirin 81 mg per day. I had a stent for a narrowed artery near the "widow maker" in September 2013. My artery was 80% narrowed this was for preventative reasons. This is an overall of my health condition at present. I would appreciate any advice that you may have. Thank You

So, you are diagnosed case of non seminoma of testis.
This cancer can spread to lungs.
And metastatic lesions in lung can be seen as nodules in their early stage.
So your current chest x ray findings of nodules, can be due to metastatic spread of non seminoma of testis.
Better to pick it up in early stage. So CT thorax with contrast is needed in your case.

So practically no difference in the approach in your case.

I have to disagree. Seminole has a distinct path from the testicle. Typically it moves into the lymph system in the lower abdomen then travels to lungs then brain. It has a distinct pattern. I have been constantly monitored with MRI and blood work along withstood markers. If this is something it would be a secondary cancer not testicular. What are your thoughts?

Yes, you are right about seminoma and its lymphatic spread.
This is predominant spread.
This does not mean that seminoma has 0% hemotogenous (through blood) spread.
Seminoma does have hemotogenous spread but predominantly it is spread through lymphatics.
Moreover lungs have their lymphatic system.
So seminoma can spread to lungs by both ways, lymphatics as well as hemotogenous.
And literature also favours this as lungs are common organ where seminoma can spread.
So I advice you to get done CT thorax with contrast to rule out metastatic nodules.
