Question: Im curious if you hace a suggestion on whatvto talk to my dr about for my options. I had synvisc one 4 weeks ago that did not help I have a cracking grinding and popping that is painful doing steps, walking getting up and down from seated position. I have had several knee surgeries over the years for torn meniscus and a Fulkerson distal patella realignment. Recently I have had a fat pad removed and Scar tissue. I have severe pain in the knee with cracking and grinding that is painful. I had an MRI and it showed chondral thinning with moderate change along the patellar Apex and medial patellar facet. Saying previous chondral plastic may be contributing. Also chondral thinning involving the trochlear groove centrally and laterally and chondral thinning and minimal marginal spurring that has progressed when compared to 2014 MRI. It also shows chondral thinning involving the medial joint compartment articular cartilage joint space narrowing

You seem to be having features of early arthritis possibly contributed by the meniscus tear and degeneration. This is attributed to the cracking and grinding noises.

You have medial joint compartment cartilage thinning which is in favour of early Osteoarthritis.

Avoid squatting, cross leg sitting, climbing stairs, kneeling, running. Stationary cycle, aerobics, swimming will help to maintain muscle bulk.

Synvisc has shown no evidence of improvement in AAOS recommendations protocol for arthritis. Steroid could help.

Must maintain your knees well because you have high chance of premature development of arthritis symptoms.

I was told that knee replacement would be my next option since I have already had the Fulkerson distal patella realignment we have tried aggressive therapy and have got no where. as the therapy hurts and is painful in the knee and behind the kneecap

Yes. That's the only surgical option left but it is too radical to be done in your case because you are young. The knee replacement implants usually have a life of 15-25 years maximum and that depends largely on your activity level and other contributing factors.

yes but what can be done to alleviate the pain I am in and have? I can't live like this I am basically immobile and can't do anything with the pain I am. is there anything left that can be done beside tkr? if I was older would that be the only option. what else can be done

You cab take Tablet Tramadol with Paracetamol combination for pain relief. Also for the other precautions mentioned earlier. Physiotherapy and range of motion exercises will help. Ultrasound therapy of the knee as well.

we have done meds and therapy as well as ultra sound and brace to help with tracking nothing has worked. therapy has been 3xs a week for almost 2 years on and off and nothing except getting worse n more pain. should I accept that I will be getting g a knee replacement? if I was say 50 or 60 would that be automatic
